Employee was in the process of removing a gearbox from a shaft. While doing this, the employee bumped their right elbow on a cross brace of the structure. The next day the employee had tenderness and a knot. The employee was seen at a clinic and returned to work. Employee was referred to ortho and seen on 7-10-25, the employee was put on restricted work.

The employee was in process of filling propane bottles and placing them on the storage rack. Employee went to place a full bottle on the rack and inadvertently caught left pointer Finger between the bottle and the rack. As a result smashing the finger and causing a laceration.
Employee was unloading a drum truck. As the employee was unloading the empty 55 gallon metal drums, they made it to the last row and the employee was pulling a drum forward and it slipped out of EE's hand, rocking back and smashing EE's thumb on the right hand.
Employee was pulling bags down off a double stacked pallet. The double stacked pallet height reach 65 inches in height. As the employee was pulling the bags off EE felt a pop, and EE's shoulder went to hurting. (right shoulder) The individual bag weight is 50Lbs per. The employee was mixing a batch mix and did not wait on the forklift to come.
Employee was replacing a bearing on ACM shaft. EE adjusted the height of the base plate for bearing. EE placed bearing and shaft into position. EE stuck EE's right middle finger into alignment hole to check position, the base plate slipped and pinned EE's finger. EE received a laceration on tip of middle finger which required stitches. Press was pulled from service.
Employee threw a 50 lb bag over the bag press at Jet Mill 2 and strained EE's back/neck. The employee was taken to local ER where a CT scan was obtained and showed two bulging discs in EE's neck. This could have been from a previous auto accident prior to EE's employment here. This was turned over to workman's comp and EE will be given an MRI.
Employee was tempting to open the bottom door of a rail car to unload material. Safety latch was stuck, when safety latch released employees finger was pitched causing tip of finger to burst. Employee was taken to emergency room and got 5 stitches.
Pulling the tin off of the south side of the building. Employee tied onto the tin with a rope so as to have more control of tin. While trying to tie the tin it started to fall and employee cut his finger.
Transporting a 1" thick steel plate with forklift. He parked the forklift next to the stands that was going to hold up the sheet. He then grabbed the sheet and started to slide it off the forks on to the support stands. The pulling motion caused stiffness and pain in his mid back region.
Employee was in the process of changing screens. He noticed that the screen appeared to be pulled more to one side. He reached his hand into the front of the shaker to check if it was still hooked properly. At the same moment the back side of the screen came off and the screen recoiled. The recoil pinned his right hand between the front hook point and the shaker wall.
Employee was attempting to straighten a 50lb packer that was sitting on a pallet that had shifted. The employee jerked on the leg of the packer and pulled a muscle in his mid back.

Twisted right ankle as employee was climbing down the extention ladder, about 4 rungs from the bottom, when his foot rolled. Ladder was reported as being wet.
Employee was picking up 25 lb. boxes & started having pain and popping sensations in his left upper back at times. He reported that he felt like he was having muscle spasms.
Employee was throwing 50 lb. bags on a pallet. Right hand got hung in the bag and the hand rolled causing his arm to roll.
Employee was reaching overhead to unhook bags. The left side of the middle of his back began to feel tight. (like a muscle spasm)
Employee was filling 35 lb. 5 gallon buckets with material. He had pain in his lower right side of his abdomen.
EMPLOYEE HAD A PIECE OF SLAG IN HIS EYE CAUSING IRRITATION. IT IS UNKNOWN HOW IT HAPPENED.
EE WAS ASSISTING CO WORKER IN RETRIEVING PART FROM TOOL ROOM WHEN A STEEL PLATE FELL OFF SHELFCUTTING HIS FOREHEAD
PICKED UP A BAG. HE WAS SACKING OFF ON A MILL AND A SHARP PAIN RAN DOWN LEFT LEG.
EE BENT DOWN TO PICK UP A BAG AND TRNED TO PUT BAG ON THE PALLET AND HE SAID HE HEARD A SNAP FROM HIS BACK.
EMPLOYEE LIFTING 50 POUND BAGS TO REPALLETIZE 1540 MATERIAL FOR SHIPMENT.
HE PICKED UP A BAG AND TURNED TO SET IT ON THE PALLET AND SOMETHING POPPED IN HIS LEG OR HIP.
EE WAS PUSHING A BAG DOWN INSIDE OF TRASH DRUM AND PUNCTURED HIS ARM WITH A STICK.
