While descending the steps of the loader, Injury hit his head on the metal support bar for the left side mirror. The mirror had been removed for repairs leaving a bare metal support bar protruding out from the top of the loader.

Driver had clot of dirt caught in the tail gate of his end dump trailer, he and the witness lifted the tail gate high enough for the dirt to fall out and the tail gate came down and caught the driver's hand between the bed of the trailer and the tail gate.

Removing rock from between tires with a piece of leap spring. While removing the rock, the tire exploded sending rock fragments in several directions. A piece of rock struck the victim on the right hand, cutting through the gloves and causing lacerations.
