EE states EE's hand was smashed by an unknown object when assisting with removal of welder from truck bed. EE stated on 8/7 and 8/8 that EE did not wish to seek medical treatment and no signs of injury were present. EE also stated that EE had previous injury to the same area of EE's hand. EE left site and sought treatment 8/9/25. EE resigned 8/20.

Showing all 31The employee was exposed to shotcrete on both legs and caustic burns at 3pm on 4/21. The employee informed the supervisor at 9am on 4/22, at which time the supervisor provided 1st aid. The employee did not mention the incident again until 6/9/25 and was sent to the doctor that day. The employee is off work until released by the doctor.
On the 19th of March the miner was part of a crew that stemmed 37 holes prior to a blast. The next morning, miner reported that miner's back hurt and the dispatcher recommended miner see a doctor. The miner went to the doctor but never reported the doctor's visit. Management found out about the restriction when they received a notification from workers compensation.
Injured employee reported a ware plate fell and hit employee's knee. Employee was released to return to work on 5/13/2022.
EE was grinding rust from the primary sidewall under the crusher. EE finished grinding and proceeded to switch from clear glasses to burning glasses. In the process, grinding material fell into EE's burning glasses. When EE put the burning glasses on, material fell into EE's left eye.
Supervisor was using a wash down hose to clear material off a conveyor. When they turned the hose on, the pressure knocked them off balance. They stepped back and stepped down a 6 1/2" step to a lower platform. This shifted all the weight to left leg injuring it. Fractured the tibia and fibula bones in left leg.
A hydraulic cylinder was being replaced on the pedestal rock breaker. The cylinder was supported with a crane and a lever chain hoist. An employee's hand was pinched in a pivot point at the rod end of the cylinder. A laceration to the employee's right hand required 9 stitches to close the wound.
The 2 employees were attempting to reposition the blow bars in the horizonal crusher, while moving the blow bars it moved and struck the employees little finger on the left hand.
While reinstalling tail pulley guard employee was trying to line the guard with the frame. The guard shifted pinching his right small finger.
Worker tossed an 8 inch 2x2 angle iron into the scrap bin, and his right hand struck the vice mounted on the back of the truck. He suffered a right hand 5th Metacarpal Fracture.
Employee was loading bundles of bolts weighing approx. 67 lbs. from a storage pallet to the loader bucket. Employee felt a pulling sensation in his back. He then visited a doctor who determined that he pulled a muscle in his lower back and placed him on light duty work
Screen rail stuck in discharge chute, struck employee's finger when it came loose.
Miner injured back lifting screen 7/9/13. Said he was fine didn't need medical treatment. Pain got worse so 7/22 took him for MRI. Results came 8/5 showing new damage to back. Put on light duty.
EE came to work on 3/30/09 and told his supervisor he was hurt. At that time, he was told to go to the doctor. He did not report any injury to Ohil Stone, Inc on 3/26/09. His Dr report says that was the injury date.
Was tearing 3' crusher apart, had to remove overflow chute. When pulling on it to dislodge the chute, rocked back onto little finger, cutting finger.
Employee broke left thumb.
Greasing loader dropped screwdriver and it rolled under front axle. Retrieved screwdriver and hit back of head on bolt on front axel.
While changing a return idler, when letting one side down the other side fell out of the bucket causing the idler to fall smashing a finger between the idler and conveyor frame.
Two employees were changing a scraper on the end 0f #10 stacking conveyor. The first one was holding the counter weight arm in place while the second one replaced the scraper blade; the arm slipped from his hand, pivoted around & struck the second EE in the mouth injuring his lower lip & chipped a tooth. Ice was used to stop bleeding & swelling.
Employee was helping another employee roll a head pulley so they could weld lagging on it. When employee pulled down, He felt something pop in his lower back.
Working on V.S.I. crusher. Inspection doors opened to access table. While ee was reaching into the crusher, one of the inspection doors fell closed striking the ee on the hard hat. The ee began having neck pain & went to get checked by a physician. The physician gave no treatment at that time. 8/9/04 ee returned to physician w/neck pain. Medication prescribed.
EMPLOYEE WAS CHANGING OUT A TEMPORARY IDLER TRAY & CUT LEFT THUMB ON METAL.
HE WAS INSIDE A PONTOON ON THE DREDGE. THERE WAS A BALL VALVE INSIDE TO BLEED AIR OUT OF THE COOLING SYSTEM. HE BREATHED IN SOMETHING WHILE BLEEDING THE SYSTEM THAT CAUSED HIM TO PASS OUT. TWO OTHER EMPLOYEES HELPED HIM OUT TO FRESH AIR AND HE CAME TO IMMEDIATELY.
EE WAS PULLING A CONVEYOR BELT ON WHEN THE BELT SLIPPED, PULLING HIS THUMB BETWEEN A RETURN ROLLER & THE BELT.
EE WAS HOSING OUT FROM UNDER CONVEYOR, FELT A BURNING IN LEFT EYE. EE RUBBED EYE & CAUSED IRRITATION. EE WAS NOT WEARING SAFETY GLASSES.
EE WAS CHANGING SCREENS & SLIPPED ON SCREEN, HIS LT. LEG HIT AND HE RECEIVED A PUNCTURE WOUND ABOUT 1/2"HOLE. AREA WAS WET.
WAS PUTTING NEW SCREENS ON DECK, SLIDING SCREEN DOWN INTO THE DECK HAND HIT TOP DECK SHARP EDGE WHERE STONE FALLS THRU SCREEN DECK, CUT RIGHT MIDDLE FINGER WAS WEARING COTTON GLOVES WENT TO EMERGENCY ROOM GOT 8 STITCHES WENT BACK TO WORK
SITTING ON THE GROUND TAKING OFF A PAIR OF RUBBER BOOTS.
REMOVING A WEDGED ROCK FROM CRUSHER CHUTE & ANOTHER ROCK CAME DOWN CHUTE, HITTING MINER ON SMALL FINGER OF LEFT HAND, CAUSING INJURY.
EMPLOYEE WAS CLEANING FLANGED AREA OF A PLANE- TARY DRIVE UNIT, TO INSTALL A NEW SEAL.
EE WAS BLOCKING THE LIME & SCREENING BIN DOORS OPEN WITH 4"X8"X16" WOOD BLOCKS TO PREVENT THE DOORS FROM BECOMING FROZEN SHUT OVER THE WEEKENDHE PLACED 1 BLOCK IN THE #8 BIN DOOR & ANOTHER E E CLOSED THE DOOR ON THE BLOCK. HE THEN PLACED 1 BLOCK IN THE #7 BIN DOOR, AND THE OTHER EE CLOSED THE DOOR ON THE BLOCK. WHEN THE #7 BIN DOOR CAME SHUT IT PUSHED THE BLOCK TOWARD THE BLOC
