As employee was replacing a 2 inch water line to the GPH3TAL, another water line ruptured with hot water and steam and hit employee in the face. Employee had his safety glasses on but sustained a corneal abrasion to the left eye. A piece of metal was removed from his left eye.

Employee alleges he aggravated a pre-existing non-work related injury to his right shoulder on 7-17-09 while setting a return roller into a conveyor frame. Employee felt okay until 7-24-09 at which time he requested medical treatment. Employee was placed on restricted work duty for a strain to his right shoulder.

Employee was injured on 6-26-09, but did not report this injury to his Supervisor until 6-29-09. Employee was turning the bottom cap of an airslide in the basement of Group 4 Silos, 26, 28, 3 0 (airslide to ME5 elevator). As he was turning the cap, it stopped while he was still applying pressure, and his thumb bent backwards.

EE was assisting the Vactor crew with a clog in the hose by lowing it over the guarding when the second employee lost grip of the hose causing it to drop down jerking the EE forward jerking his shoulder and head which caused strain to his back. EE did not seek medical attention until 05/22/06.

EE called in on 5-04-06 said he hurt, complained of whole body sore. Was informed could not return to work - neeeded doctors release. Did not mention an on job injury. 05-05-06 EE reported at 3:30 PM a fractured toe from rebar received on 5-03-06. No doctors report.

EE WAS REMOVING BOLTS FROM THE CUTTING EDGE OF THE 988G LOADER BUCKET IN DOING SO A SHARP EDGE ON ONE OF THE BOLTS CUT THROUGH THE EMPLOYEE?S GLOVE AND INTO HIS LEFT INDEX FINGER. THIS REQUIRED THAT THE EMPLOYEE HAVE SIX STICHES PUT IN HIS INDEX FINGER .

Employee was closing water valve on K3 water tank. The valve became stuck and the employee pulled hard to loosen valve, straining his back and shoulders.
EMPLOYEE WAS CLEANING HOT DUST WITH BOBCAT WHEN BOBCAT STALLED. EMPLOYEE EXITED EQUIPMENT AND STEPPED INTO HOT KILN DUST AND RECEIVED SECOND DEGREE BURNS OF THE FOOT.

AS BELT WAS PULLED TIGHT IT JUMPED OVER SMALL RETAINING WALL AND STRUCK EE ON OUTSIDE OF LEFT KNEE. MEDICAL EVALUATION SHOWED STRAIN. EE MUST WEAR KNEE BRACE AND WILL BE ON RESTRICTED DUTY UNTIL COMPLETION OF PHYSICAL THERAPY.
