Maintenance employees were loading a feeder onto a flatbed trailer. The flatbed trailer had raised wheel wells. As the feeder was being lowered, a Maintenance Helper used his right hand in an attempt to keep the equipment from swinging. As the equipment lowered, the Maintenance Helper's right fifth finger was pinched between the feeder leg and the trailer fender.

Showing all 15Employee was unloading a roll of conveyor belt from the back of a van. When the employee and co-workers rolled the belt out of the van, the belt hit the ground and tipped sideways, striking the employee in the back of the left leg resulting in a bruise.
While setting an oxygen cylinder onto the back of a service truck, the employee pinched his middle finger between the cylinder and a spud bar which was laying on the floor of the truck, resulting in a fracture to the tip of the middle finger of his left hand.
EE had a stone fall on his leg in the palletizing area.

EE WAS HELPING HIS CO-WORKER WITH SCREENS ON TOP DECK OF 6X16. HE WAS GRABBING THEM AS CO-WORKER WAS HANDING THEM DOWN. HE CAUGHT HIS RIGHT RING FINGER BETWEEN SCREEN AND EYEBOLT ON SHAKER. FI NGER WAS SMASHED, LACERATED, AND BROKE TIP OF FINGER. 3 STITCHES TO CLOSE WOUND.
EE WAS SHUTTING THE DOOR OF THE CAT TRUCK, HIS HAND SLIPPED OFF OF THE HANDLE & HE GOT IT CAUGHT ON THE LATCH SHUTTING HIS HAND IN THE DOOR.
OVERHEAD CAGE PINCHED MY LEFT RING & MIDDLE FINGER IN THE HINGE WHEN I WAS PULLING IT DOWN. THIS CAUSED A LACERATION TO MY MIDDLE FINGER AND PARTIAL AMPUTATION OF MY RING FINGER (HALF-WAY DOWN TO THE FIRST JOINT). THIS LEAD TO NO USE OF MY LEFT HAND & SEVERE PAIN. GLOVES WERE BEING WORN AT THE TIME OF THE ACCIDENT.
EE STARTED TO FELL PAIN IN HIS LOWER BACK WHILE STACKING BAGS OF LIME. THE EE WAS TWISTING HIS BACK WHEN MOVING THE BAGS.
AFTER EE REMOVED LAST BOLT OUT OF A PAN IT SLIPPED PINCHING HIS LITTLE FINGER ON HIS RT. HAND. HIS X-RAY WAS POSITIVE FOR A SMALL HAIR LINE FRACTURE. TREATMENT CONSISTED OF TAPING ADJOINING FINGER TO INJURED DIGIT FOR SUPPORT.
