Showing all 17Employee was removing worn paddles from material washer by lifting paddles to edge of tub and tipping them over the edge. Employee lifted paddle to edge and didn't clear lip of tub causing the paddle(40lbs) to fall approx. 10in. smashing employees right middle finger. Resulting in fracture to tip of right middle finger. Management notified of incident on 4/25/2016 @ 8:30am.
Employee was operating a Cat TH83 forklift to remove a 6" steel pipe from a walkway. While moving the pipe, a second employee came over to direct the operator. The 6" pipe was lowered to the ground when it pinched against the mast of the forklift, causing pipe to be launched into the air, causing a fracture to the left foot.
Employee was helping a heavy duty repairman install a (2) section belt clamp on a conveyor belt. EE did not realized his right middle finger was between the belt clamps and when the repairman tightened the side tension clamps with an impact guy, his finger was caught between the upper and lower belt clamps.
Employee was performing normal duties of a weighmaster which included tearing ticket tags for customers, when she developed swelling and pain in both wrists. When reported to the company, the employee was given modified duties that were accommodated until 2/17/12. Her condition did not improve so modified work was ceased to allow for additional healing.
Employee was moving coarse material washer shoes throughout his shift from pallet lifted by forklift and handing to another repairman before they were bolted into place on flights of shafts. Shoes weigh 31 lbs. each. At the end of the work shift the employee's right shoulder was stiff and painful.
Employee was hooking up a suction hose from a tank to the pump for the water that comes off of the sand dewatering screen. He was leaning over to put the suction hose into place, when he felt a small pinch in the lower middle back.
The employee was working on a feeder to install a skirtboard. While cutting the skirtboard through material with less force, the right hand impacted the sideboard pinching the small finger and cutting the thumb.
WHILE SHOVELING, EMPLOYEE'S KNEE BECAME INCREASINGLY SORE. EE WENT TO THE CLINIC & WAS GIVEN MODIFIED DUTY FOR 3 DAYS.
AFTER WORKING LONG SHIFTS FOR OVER 2 MOS. THE EE COMPLAINED OF PAIN IN HIS RIGHT SHOULDER. HE TRIED TO WORK THRU IT, PAIN DID NOT GET BETTER. MD DIAGNOSED RIGHT TRAZPEZIUS STAIN & GAVE HIM AT LEAST 2 WKS OFF.
WHEN EMPLOYEE CARRIED A HEAVY OBJECT ON HIS SHOULDER HIS ARM WENT NUMB. ARM AND HAND KEPT "TONGLING" AFTER HE PUT LOAD DOWN.
FOR THE LAST COUPLE OF MONTHS THE EE BACK WAS SORE. HE FINALLY WENT TO THE DR ON 11-6-01. THE DR PUT HIM ON MODIFIED WORK.
EE TWISTED HIS KNEE WHILE STANDING ON THE TRACKS OF A BULLDOZER. THIS OCCURRED ON 5-8-01. HE CONTINUED TO WORK BUT KNEE DID NOT SET BETTER. HE FINALLY HAD AN MRI DONE ON KNEE AND A LIGAMENT TE AR WAS FOUND. HE WORK UNTIL 6-11-01 WHEN HE WAS PUT ON A NO-WORK STATUS BY HIS DOCTOR. EE WAS GREASING DOZER AT THE SHOP.
EE STRAINED HIS BACK WHEN HE TRIED TO LIFT A 55 GALLON DRUM TO THE UPRIGHT POSITION.
WORKER WAS SWINGING SHAFT THAT WAS HANGING FROM CRANE TO TIGHTEN NUT ON HP300 CRUSHER WHEN HAND GOT CAUGHT BETWEEN SHAFT AND NUT IMPACTING HAND.
EMPLOYEE WAS WINDING A HOSE ON A SPOOL. AS EMPLOYEE TURNED THE HANDLE ON THE SPOOL RAPIDLY, HE STRUCK HIS HAND ON THE EDGE OF A TOOL TRAY. HE CUT HIS THUMB AND LOST HIS FINGERNAIL.
EE WAS INSTALLING GUARDRAIL. THE RAILING WAS NOT ATTACHED YET. RAILING STARTED TO FALL. EE GRABBED END OF RAILING. RAILING VIOLENTLY SHOOK EE'S ARM WHEN THE OTHER END HIT THE GROUND. END OF RA ILING FELL ABOUT 6'. EE SUFFERED SPRAIN TO LEFT BICEP AND STRAIN TO LEFT SHOULDER.
