As worker was climbing in to the flatbed of a truck ee felt a pop in left foot. EE continued to work and declined treatment at the time of injury. EE did not think it was too bad and thought it would get better. Pain gradually worsened over several days. On 6/11/17 ee was hurting bad and decided ee needed treatment.

Showing all 4Employee stated that the line on the capson head got too tight. When he went to take off the line it jumped and hit the palm of his left hand and jammed his wrist.
Employee stated that there were two possible causes. Either when he was installing a plate in a chute or when he was pushing a guard in place. Right side of ribcage sore.
EE was moving a shaft from skiff to the back of the pickup & slipped.
