Employee was stepping down from the John Deere Backhoe when he felt a sharp pain/popping in his left hip.

Employee was hit in the mouth with a jack, knocked out tooth.

Employee pulled/strained back while removing a large piece of coal from the feeder.

Employee injured back when he reached over a vibrator to pick up a header block.

Contract employee pulled shoulder and neck muscle while removing a bar from a belt roller.

Employee was lifting pulling a piece of belt structure when he felt a sharp pain in his neck. Pulled muscle.

Employee pulled lower back muscle when he stooped over to replace his tracking reader battery.

Employee was lifting and pulling. Replacing the back on a flute liner for a pump. He felt a sharp burning pain in his stomach area.

Ee was stretching to lift cable high enough for bolter to go under and pulled muscles in his abdomen. Took two days off and returned to work with no problems.

Ee was working on a starting box on a scoop. As he lifted part of the apparatus, he strained his lower back. He is under doctor's care and still off work.

Ee was greasing a drive line when the coupling came loose and shot grease into his hand. The grease actually penetrated his skin. He knew to see doctor's care immediately. With antibiotics his hand healed very well while he only missed one day.

EE was climbing onto side of bolting machine to install glue and bolts, when he pulled a muscle. Ended being very minor with no long term effect at all. He only missed 8 work days.

Ee is a miner helper. He was moving miner cable from the roadway when he pulled muscles in his back. He only missed two days and came back to work with no problems.

Ee was on his knees changing a bottom roller. When he reached for the roller his back "gave out". He is still off work as of 1-12-12 and in physical therapy.

Ee was pulling a 10 foot bolt when he strained his groin. Doctors have him off work. They don't know yet if it is a hernia or just a muscle strain.

Ee was installing a V-belt on a motor when his finger got caught. This mashed and cut off the end of his finger. Took a few stitches and Ee missed two days work (Friday and sat).

EE was pumping water. His hand slipped on a 2' water clamp and cut his hand. No problem, EE went back to work next day.

EE was shoveling the belt line, when a splice came by and cut his arm.

Ee cut his finger on a best conveyor shield. It took two stitches and Ee returned to work on the next scheduled work day.

EE was installing ridged rail belt structure. He was lifting with both hands when he felt a strain in his groin. He tried to work thru this for weeks and weeks, but then decided to consult a doctor, who found he had a hernia. His first day off work was 9-21-10. He is doing well now.

Ee was working on a motor, when his thumb got caught between motor and support. Cut his thumb, required stitches but did not miss any work.

EE was cleaning a roller with a piece of angle iron. A frozen piece of mud caught the angle iron, kicked back and mashed his hand against the structure. X-rays revealed no broken bones but Ee missed one day of work.

Ee was unloading rock dust from a belt line and twisted his back.

Ee was taking apart a frozen water line. As he was pulling it apart it flew up and hit him in the nose. This broke a vessel and caused extensive bleeding. He had to have surgery to final take care of the problem.

The crew was lifting the belt rope to install new bottom structure, when ee's back "went out". He was diagnosed with muscle strain and came back to work in six days.

Ee was shoveling when he pulled something in his back. It was only a pulled muscle. Ee went back to work, with no restrictions on 4-30-08

EE was lifting battery covers and felt something pull in his back. He tried to work it out, but had to go to doctor two weeks later.

Ee was working on brattices. He had removed part of one. Unknown to him, a block had stuck to the top until he got under it and it fell, hitting him in the back.

Ee was tightening wheel brake on a rail road car and popped his shoulder out of place.

MINER WAS LIFTING A CAN OF OIL AND STRAINED HIS LOWER BACK.

EE WAS LIFTING A SHUTTLE CAR REEL PULLED MUSCLE IN LOWER BACK

EE WAS BUILDING A STOPPING UNDERGROUND BANGED HIS HAND. THE MIDDLE KNUCKLE ON HIS RIGHT HAND WAS SWOLLEN. HE WENT TO DOCTOR THE NEXT DAY AT 6 PM TO GET IT CHECKED OUT.

EE WAS STEPPING AWAY FROM A ROOF BOLTER AND HIT HIS KNEE ON THE MACHINE CAUSING BRUISING AND STIFFNESS.

WHILE REMOVING ROCK FROM BELT LINE, EE TWISTED HIS BACK.

MINER WAS UNHOOKING A PIECE OF LOW LOW STRUCTURE AND A PIECE OF METAL FROM THE STRUCTURE WENT INTO HIS FINGER. IT WAS A SMALL CUT TO HIS LEFT MIDDLE FINGER, BUT IT WAS DEEP AND HAD TO BE REMOV ED BY A DOCTOR.

EE STRAINED BACK WHILE SHOVELING COAL.

EMPLOYEE WAS BUILDING A BRATTICE AND TWISTED HIS BACK.

EE WAS UNLOADING STRAPS OFF SCOOP & STRAINED HIS BACK.

EMPLOYEE WAS MUCKING BELTS AND HIT A PIECE OF STRUCTURE WITH HIS RIGHT WRIST.

EE DEVELOPED PAIN IN LOWER BACK WHILE PULLING MINER CABLE.

EMPLOYEE WAS PUSHING OPEN A MINE DOOR & SPRAINED HIS LEFT SHOULDER.

EE WAS REPAIRING BROKEN BELT LINE & CAUGHT LEFT RING FINGER BETWEEN BELT STRUCTURE & TIMBER.
