Employee had laid the master pad for a Caterpillar D-9 dozer in place and turned to get a pry bar and the pad fell and struck his right foot.
While putting a man door in the stopping a block fell striking his left hand requiring stitches.
Employee had been driving a ground rod and as he was catching his breath, he leaned on an old stacker belt assembly (the stacker assembly did not have any power source attached) and his hand moved the poly-belt attached between the motor and speed reducer resulting in his right ring finger being caught between the pulley and belt causing a fracture to the tip of his finger.
Employee was removing a piece of rope from a return roller on the beltline when the rope caught pulling his left hand between the roller and belt resulting in a fracture to the tip of his ring finger.
Employee was dropping a S/C anchor when he twisted and pulled something in his back. Said he felt a sharp pain in his lower back when he pulled on the cable for slack.
Employee was loading roof bolts and plates onto the tray of the roof bolter when his right hand was lacerated on a roof bolt plate requiring stitches.
Employee was removing a rock from the #2 take-up when he felt a pain in his shoulder, back and abdomen. He continued to work until 10-3-11 when he started missing work. Workers Comp was looki ng to see if a pre-existing condition existed and the Safety Dept. was notified on 11-2-11 that the accident did not involve a pre-existing condition. Diagnosed with a hernia.
Employee was connecting black water line together when the water line slipped out of the coupling striking the employee in the mouth, breaking a tooth.
Employee was attempting to clean his drill steel, when he struck it against the pot it bounce back striking employee on the nose causing a 1/4 to 1/2 inch laceration that required 2 to 3 stitches. Employee was transported to HRMC ER at Prestonsburg KY, and was released later that evening.
Employee was pulling metal bands off of a bundle of roof bolts when band pulled free it struck employee in arm resulting in a laceration.

EE was helping to install a tilt cylinder on a Komatsu loader. As he was trying to align the cylinder with the mounting bracket it was moved by the boom truck operator and he stated he was jolted, causing pain in his lower back. EE continued to work until 3-22-11.
EE is 72 inches tall, the seam height is about 60 inches, and the floor is relatively dry. EE was attempting to move the miner cable and waterline, while moving waterline/miner cable, EE felt a pain in his lower back and heard a pop in his back.
EE stated he was in the process of loading a new scoop door onto a 4 wheeler at the end of the track, while loading the scoop door, EE felt pain in his lower back.
Member was removing top structure and dropped structure on left middle finger. Went to hospital after work and recieved 10 stitches.

Employee had walked the main belt-line and came outside. He was at the drift mouth at A-1 belt. Employee bent over to pick up a piece of chain link wt. about 20 lb. He developed a sharp pain in his low back. Dr. at the E.R. examined him and said he had acute muscle strain, told follow up with family Dr., he saw family Dr. 8-5-09, and was taken off work. (Mine Closed 2-10-10)
Employee was closing the guard on the breaker box of a scoop, caught finger between guard and lid cutting middle finger, Requiring 4 stitches.
Employee was working on the pad of the Highwall miner. A car was placed on the pad and the employee and another pad man coupled up the cat heads on the car. The employee went back to pull the slack back into the tray resulting in pain in the lower back.
Employee was unloading a deck bushing for a drill from a pickup truck to the mechanics drop when he dropped the part. When he bent over to pick up part he felt a pain in his lower back.
EE WAS BUILDING A STOPPING. IT WAS LEANING VERY BAD AND WAS NOT WEDGED. STOPPING STARTED TO FALL OVER, HE TURNED TO GET OUT OF THE WAY AND TRIPPED. A BLOCK FROM FALLEN STOPPING HIT HIM IN THE SHOULDER.
CARRYING A 50 LB BAG OF FLOCK UP A HILL. HE EXPERIENCED PAIN IN HIS NECK & SHOULDER. 1ST WORK DAY MISSED 6-25-03.
CHANGING BRAKE CALIPER ON SERVICE TRUCK. TRUCK WAS HELD UP WITH A FORKLIFT. TRUCK SLIPPED OFF FORKS, CATCHING HIS RIGHT HEEL WITH STUD BOLT THAT HELD CALIPER ON.
EMPLOYEE WAS PACKING 15' STEEL BEAMS TO SET IN CRIB TO SUPPORT THE TOP. HE WAS STANDING ON BELTLINE PUSHING BEAMS INTO PLACE WHEN HE ALLEGES HE FELT HIS SHOULDER PULL, WHEN HE DID IT FOR THE S ECOND TIME HE SAID IT MADE THE PAIN WORSE.
PUTTING TOP AND BOTTOM BELT STRUCTURE IN, WHILE MAKING A SECTION MOVE, INJURED LOWER BACK.
EE WAS LOADING 8' ROOF BOLTS ONT HE ROOF BOLTER WHEN HE FELT A PAIN IN HIS MIDDLE BACK.
EE WAS REPLACING CLEAN COAL SCREEN, WHILE INSTALLING SCREEN EMPLOYEE FELT PAIN IN GROIN AREA.
EMPLOYEE AND 2 OTHER EMPLOYEES WERE HANGING CM CABLE. LATER IN THE SHIFT EMPLOYEE NOTIFIED SECTION FOREMAN THAT HIS BACK WAS HURTING. EMPLOYEE STARTED MISSING WORK ON 6/27/02. MUSCLE STRAIN IN BACK.
EE WAS PICKING UP MINER CABLE SO THAT THE ROADWAY WOULD BE CLEARED AND FELT A SHARP PAIN IN HIS LOWER BACK.
EMPLOYEE HAD POSITIONED CRIB BLOCK UNDER GATHERING POT. PAN WAS BEING RAISED UP AND DOWN ON CRIB BLOCK. TRYING TO BREAK GATHERING POT LOOSE. EE REACHED IN TO RE-POSITION CRIB BLOCK WHEN PAN WAS LET DOWN ON HIS LEFT THUMB, BREAK AND LACERATION TO LEFT THUMB.
EMPLOYEE WAS LOADING 7 7/8' DRILL INTO PICK UP WHEN HE FELT PAIN IN STOMACH, SMALL KNOT AROUND NAVEL AREA.
EE WAS HANGING MINER CABLE WHEN HE FELT PAIN IN HIS BACK.
EE & 2 OTHER EE'S WERE IN TRACK ENTRY PREPARING TO HANG HI-LINE. NO WORK WAS BEING PERFORMED AT THIS TIME. EE SAID HE FELT PAIN IN HIS NECK. EE RET'D TO WORK ON 2/26/02, MISSED 2/27/02 CALLED IN ON 2/28/02 SAID OFF TILL 3/4/02 DUE TO BREATHING PROBLEMS
EE WAS MOUNTING A ROCK TRUCK, FIRST STEP WAS A CABLE STEP, EE PLACED RT. FOOT ON STEP THEN PUT HIS WEIGHT ON THE STEP. WHEN DOING SO THE STEP SWUNG CAUSING PAIN IN RT. KNEE.
INJURED WAS CLEANING A SIEVE SCREEN HOLDER FRAME. WHILE CLEANING HE HELD ON TO A SUPPORT BRACLED BAR BRAKE LEAVING A SHARP EDGE THAT CUT THORUGH HIS GLOVE AND MADE A SMALL CUT IN THE PALAR OF HIS RIGHT HAND, REQUIRING 3 STITCHES.
EE WAS LOADING 5' RESIN RODS INTO THE SUPPLY TRAY ON THE SIDE OF A ROOF BOLTING MACHINE. HE PLACED 6 OF THE RODS INTO THE TRAY AND AS HE WITHDREW HIS HANDS/ARMS HE STRUCK THE TOP OF HIS RIGHT HAND AGAINST A 9 1/2" METAL BRACE ON THE SUPPLY TRAY RESULTING IN A CONTUSION. X RAYS WERE NEGATIVE BUT THE EE FAILED TO REPORT FOR WORK ON HIS NEXT REGULARLY SCHEDULED SHIFT 3-21-01.
EE WAS PULLING FIBERLOCK OF SLURRY PIPE IN ORDER TO COUPLE PIPES TOGETHER WHEN HE PULLED TOO HARD STRAINING THE MUSCLE. HE DIDN'T MISS ANY WORK UNTIL ON 11/1/00 HE ONCE AGAIN WAS INJURED. HE W AS REMOVING A PIECE OF METAL FROM HIS TRUCK. WHILE HE WAS PULLING ON THE METAL HE REINJURED HIS RIGHT BICEP.
WHILE INSTALLING A HYDRAULIC PUMP ON A D10N BULLDOZER. EE FELT A SHARP PAIN IN HIS BACK WHILE POSITIONING THE HYDRAULIC PUMP.
