Employee had been driving a ground rod and as he was catching his breath, he leaned on an old stacker belt assembly (the stacker assembly did not have any power source attached) and his hand moved the poly-belt attached between the motor and speed reducer resulting in his right ring finger being caught between the pulley and belt causing a fracture to the tip of his finger.

Employee was removing a rock from the #2 take-up when he felt a pain in his shoulder, back and abdomen. He continued to work until 10-3-11 when he started missing work. Workers Comp was looki ng to see if a pre-existing condition existed and the Safety Dept. was notified on 11-2-11 that the accident did not involve a pre-existing condition. Diagnosed with a hernia.

Employee was attempting to clean his drill steel, when he struck it against the pot it bounce back striking employee on the nose causing a 1/4 to 1/2 inch laceration that required 2 to 3 stitches. Employee was transported to HRMC ER at Prestonsburg KY, and was released later that evening.

EE was helping to install a tilt cylinder on a Komatsu loader. As he was trying to align the cylinder with the mounting bracket it was moved by the boom truck operator and he stated he was jolted, causing pain in his lower back. EE continued to work until 3-22-11.

EE is 72 inches tall, the seam height is about 60 inches, and the floor is relatively dry. EE was attempting to move the miner cable and waterline, while moving waterline/miner cable, EE felt a pain in his lower back and heard a pop in his back.

Employee had walked the main belt-line and came outside. He was at the drift mouth at A-1 belt. Employee bent over to pick up a piece of chain link wt. about 20 lb. He developed a sharp pain in his low back. Dr. at the E.R. examined him and said he had acute muscle strain, told follow up with family Dr., he saw family Dr. 8-5-09, and was taken off work. (Mine Closed 2-10-10)

Employee was working on the pad of the Highwall miner. A car was placed on the pad and the employee and another pad man coupled up the cat heads on the car. The employee went back to pull the slack back into the tray resulting in pain in the lower back.

HE PLACED 6 OF THE RODS INTO THE TRAY AND AS HE WITHDREW HIS HANDS/ARMS HE STRUCK THE TOP OF HIS RIGHT HAND AGAINST A 9 1/2" METAL BRACE ON THE SUPPLY TRAY RESULTING IN A CONTUSION. X RAYS WERE NEGATIVE BUT THE EE FAILED TO REPORT FOR WORK ON HIS NEXT REGULARLY SCHEDULED SHIFT 3-21-01.
