Miner was cutting split-set bolts using a reciprocating saw, wearing goggles - got metal particles around the eyebrows. Washing face at end of shift a metal particle transferred from eyebrows into left eye but he noticed no discomfort. The next morning he thought he had an eye infection, called in sick and sought treatment where metal particle was found.

Employee was moving a jackleg from a draw point to another draw point while lifting the jackleg he felt a slight pain to his neck/shoulder area. Employee was evaluated by doctor on 12-20-11 (MRI negative) with a second evaluation on 12-29-11 (MRI) will be done.

The gear puller was attached & pressure was applied, when employee used a crescent wrench to hold the shaft from turning as the pressure was applied. One of the gear puller arm broken & puller sprung away from the pinion gear with force, causing the force pulled on the arm or struck left shoulder.

Employee was holding the lowboy hitch while loci operator positioned the loci for hook-up. The loci jumped when the motion was engaged. The loci struck the end of the hitch which then popped upward striking the employee in the chin, no days away from work.

The EE complained of sore back 9/27/11 & declined medical care. On 10/3/11 he was taken to clinic for pain and clinic diagnosed inflamed muscles & released to full duty. MRI was completed on 10/6/11 and physician transferred EE to original physician in Idaho due to pre-existing injury. 10/9 EE went to Idaho to see his dr and is still on full duty.

Employee was using "floor dry" material to absorb oil on floor-when he felt something in his eye. He was sweating and wiped his (L) eye w/ paper towel and his eye became irritated. Employee was diagnoses with scratched cornea to left eye. No lost time, no restricted work, medical treatment only.

During the drilling for ground support, while holding steel with left hand and the jackleg in an upright position with his right hand, employee pushed drill steel up, and immediately felt pain on his left shoulder. Employee did not miss work and incident was not reportable injury until surgery, employee's first day away was 4-11-11.

Employee was lifting a 5 gallon bucket with 4 gallons of water in the bucket. As the employee lifted the bucket, they felt a sharp pain in their lower back. As a result the employee re-injured his back from an off the job injury that resulted in back surgery earlier this year.

Employee reported that he strained his neck and felt it several times before but more so on 7-2-10 after unloading rebar. He reports the strain is from 2008 injury. Employee did not report until 7-7-10; took vacation, then sought his own medical care, did not report to employer then. He was evaluated by doctor & placed on light duty. This injury may be linked to 2008.

Employee was working on a water truck and was using the tailgate to access the rear axle box. Employee was climbing from the ground into the back of their pickup when the employee felt their right knee pop causing pain to the employee. Employee had surgery on 8/20/10 to repair a torn meniscus and returned to work on 8/23/10 with work restrictions.

Employee was tapping mounting holes so new bolts would thread easier when mounting a new starter. The employees head was directly under the mounting holes when a piece of foreign material fell in his right eye. The employee used eye drops and thought he flushed it out. The next day the mechanic went to a doctor and a piece of metal was removed from eye.

During Longwall recovery, employee was helping pull hoses off face with the use of a scoop and chain. The chain slipped off the end of the hose, causing the hose to spring back and strike the employee in the leg.

EE was removing rocks from the draw off tunnel when he lost grip, causing right hand to be caught between the rock and concrete floor in the tunnel. 1st aid was given, seen at ER, diagnosed with a hair-line fracture of his little finger.

While installing the saddle block at the load zone on a belt, the saddle fell over coming into contact with the EE's legs. Intial diagnosis was a first aid, however after continuing pain, the employee had an MRI done which showed a fractured left tibia.

While waiting for the employees shift to start the employee started cleaning up her reporting area. While using a shovel to move garbage, she felt a sharp pain in her hip. The employee went to the doctor and was not released to return to work because of the hip injury.

Injured employee and three other coworkers were preparing to build a brattice. Injured employee was taking the plastic shrink wrapping from a pallet of blocks. As employee was removing wrapping, a block became dislodged from the pallet, fell, and struck employee on the top of the right foot, resulting in a contusion.

The employee alleges while lifting a 4" water hose to attach to a valve on a truck he pulled a muscle in his arm. The employee continued to work with no restrictions from the doctor until his surgery on August 15th, 2008.
