Employee was assisting in unloading supplies when a pallet of block on the fork lift hit the water line causing it to fall, striking the employee on the left ankle and track rail.
Employee was removing steel wedges from the outer cutter drum on a continuous miner. While EE was pulling one, the adjacent one flew out striking EE in the upper lip resulting in 8 stitches.

While replacing guarding after replacing tail roller @ tail of plant feed Belt. The guarding fell catching employees left Middle finger between the guarding and structure: Employee received medical attention where he had Debridement left middle finger, local tissue re-arrangement.
While pushing 6' bolt, using the kicker on the side of the drill pot. The bolt bent in the center, smashing his right little finger. He received two (2) stitches in his fingernail bed.
While rock dusting, using a 4-wheeler and a pull behind duster. The duster became lodged on a timber near the roadway pulling the duster off the 4-wheeler. Employee while attempting to reconnect the duster lost his footing and fell with his left hand under the goose neck causing a laceration to his left index finger. Resulting in amputation from surgery.
Injured was hanging miner cable with 2 other employees, when he felt what he described as a pop in his groin area.
Employee was moving top structure out of the way, stacking it against the rib. A piece of structure Employee just stacked fell over striking his right middle finger resulting in a fracture and seven (7) sutures. On April 1, 2015 Employee had surgery to repair right middle finger.
While Employee was shoveling material he felt a pain in his lower back.

Employee was helping unload high voltage cable when a loop came down striking Employee in the face. An internal investigation has been conducted and has been waiting on medical documentation.

While hooking up cathead, Employee had right hand under cathead. When moving cathead to latch, the latch pin bent and caused cathead to smash finger between cathead and power center (Fracture - Seven (7) Sutures).

Employee was installing fence guarding under the plant feed belt. Employee kneeled down and reached out to pick up one end of a fence panel and felt a pain in his back. Employee was treated at CAMC Memorial Division where he received injections. Employee began missing work 3/17/14 and did not contact Management or Safety to inform us of his medical treatment.
While Employee was helping load 10' roof jacks, Employee felt a pain in his lower back. Employee was treated at Raleigh General Hospital. The Safety Department has learned of Employees physical therapy treatments which occurred during February 25, 2014 through April 8, 2014.
Employee felt a pop in his right elbow as he was throwing a piece of curtain on the flat car. There was a piece of shuttle car cable tangled in the curtain. Employee was treated at MedExpress and x-rays indicated joint effusion with a possible fracture.

Injured was unloading chuck bolts from the back of the bolt machine on the Number 2 section when his left arm caught a 4' strap causing a laceration his forearm the acting. Injured went to medical facility and received ten sutures.
Lifting & lowering 100 & 150 ton hydraulic jacks inside the car body of a 495 shovel. The space is very tight, working in awkward positions. The jacks weigh somewhere close to 100lbs or more.
EE was dismounting equipment. 1st step to ground, stepped on a rock causing him to loose balance. Trying to regain balance he slipped in gravel and fell to ground. Placed his right arm out to break the fall and injured the right wrist. Saw a doc on 7-19 and x-ray was positive. Brace was applied. Cast was applied on 7-23. No time has yet been lost.
EE was setting 3 inch pump on bottom step of steam truck and had to reposition himself to move pump up to the bumper of truck. He then picked up the pump and felt pain in his lower back.

Employee stated that he was loading 50ft rolls of water line onto the back of a flat bed truck when he felt something pull in the lower abdomen area.
Injured was walking along beside miner, sliding his hand along the miner when his Right hand caught a burr cutting his hand below right thumb resulting in 6 sutures.
Injured was helping load supplies, picked up bundle of plates and cut through right glove and cut palm where little finger meets hand.(Received 8 sutures)
Changing out a bottom roller on a conveyor belt and hurt his back.
He was overloaded and had to drop off while pulling on tailgate his right hand got caught between the latch and pin.
White was pulling a pump out of a roadway and felt pain shoot down his lower back. **Addendum ~ Employee did not start missing work until 3/9/2013**
Employee was loading track rails from the old works and loading them on a rail car. They had a couple rails on the rail car that needed adjusted to lay right in the rail car. Employee grabbed the rails to move them, he grabbed the bottom rail to slide it down when the top rail rolled over pinching his left middle finger.
Employee was loading top belt rollers into the scoop bucket and heard a pop in his right shoulder, followed by pain. This is an old injury that he has been having issues with.
A load of track bars came in at the high line crews office and they were falling off the truck. EE went down there to reload them. As he was reloading them, he had slide his hand down the bar when a sliver of metal punctured his left hand.
Employee shoveling #4 tailpiece and twisted to throw coal on belt without turning his entire body. He felt pop in left knee. Previously hurt knee 8/15/12 but it was file only. He is off with torn ligaments.
