ROOF BOLTER OPE9ATOR WAS LIFTING A 4"X8"X 16" COLLAR AND DROPPED ONE END OF IT ON HIS RIGHT FOOT.IT ONLY MADE A RED PLACE ON TOP OF HIS FOOT.
I.NAME WAS PUTTING SUPPLIES ON THE ROOF BOLTER. HE PICKED UP TWO BUNDLES OF BOLTS AND BENT OVER TO PUT THEM ON THE MACHINE AND HURT HIS BACK.
WE HAVE NO KNOWLEDGE OF THIS INJURY. WE HAD GIVEN THIS MANA SUSPENSION SLIP AND 10 MINUTES AFTER WE GAVE IT TO HIM. HE SAID HE HAD HURT HIS BACK WHILE HANGING A CABLE.
