Employee was taking driveline off #305 685 Komatsu Truck. Employee's hard hat fell off and he bent down to pick it up and driveline fell out striking the employee in the top of the head causing a laceration. Employee went to Logan Regional Medical Hospital and received two staples to his head and was released to return to work.

Attempting to attach cables to a bucket of a Komatsu/Demag hydraulic excavator, technician exerted a strain on the cable unaware that the cable hook was caught beneath the bucket, he applied a greater pull on the cable, to free the cable, he experienced an acute pain in the lower lumbar region of his back beginning at his tail bone radiating up the center of his pelvis.
Was installing a solid metal bar on rear axle,wasn't fitting correctly, other tech let go of his end and EE was trying to turn his end and it slipped out and smashed hand.

Employee was preparing to use a porta-power with ram and jack spacer to push bucket pin from 994 loader. He had leaned the jack spacer piping end-to-end against the loader frame when it fell on top of his right hand.
EE stated that while mounting the 'first step' of #518 dresser truck, he felt a pain in his left side. EE continued to work, but started missing work on October 27 to seek medical attention.

EE STATED THAT AFTER CHECKING OIL IN ROCK TRUCK HE PUSHED THE ENGINE OIL ACCESS LADDER BACK INTOPOSITION AND FELT A STRAIN IN HIS GROIN AREA. EE CONTINUED TO WORK FOR SEVERAL DAYS AFTER ALLE GED INJURY. ON 10-22-03 DR'S OFFICE INFORMED EE THAT EE WAS RESTRICTED FROM WORK.
EMPLOYEE WAS IN THE PROCESS OF LOADING OXYGEN TANKS ONTO HIS WELDING TRUCK. HE STATED THAT WHILE LIFTING ONE OF THE TANKS UPRIGHT HE FELT BACK PAIN. *NOTE: EMPLOYEE WAS REMOVED FROM WORK AFTER VISITING DOCTOR ON 4/8/03.
WHILE PUTTING ON SAFETY GLASSES, SOMETHING FELL IN EYE.
EE WAS MOVING RIM PARTS AROUND IN TIRE LOT. 57 INCH BEAD SEAT BAND FELL OVER STRIKING HIM IN THE BACK OF LEFT LEG AT CALF, CAUSING BRUISING & CONTUSION.
EE WAS SITTING IN THE OPERATOR'S SEAT ON THE DRAGLINE./ HE REACHED UNDER THE FRONT PORTION OF THE SEAT TO SLIDE IT FORWARD WITH HIS RT. HAND WHILE SIMULTANEOUSLY LOWERING THE SEAT WITH HIS LEF T HAND ON THE SIDE CONTROLS. AS THE SEAT LOWERED, THE FRONT ADJUSTMENT BAR PINCHED THE TIP END OF HIS RT. MIDDLE & RING FINGERS.
EMPLOYEE AND FELLOW WORKER WERE REMOVING RADIATOR FROM LOADER WHEN THE RADIATOR DROPPED SLIGHTLY, MASHING THE TIP OF EMPLOYEE'S FINGER BETWEEN THE RADIATOR MOUNT AND LOADER FRAME.
EMPLOYEE WAS IN THE PROCESS OF INSTALLING HIGH VOLTAGE COLLECTOR RING SEGMENT WHEN HE CUT HIS HAND ON SHARP EDGE OF THE SEGMENT. EMPLOYEE WAS NOT AWARE OF BEING CUT UNTIL HE LATER NOTICED BLOO D ON HIS HAND.
