HE WAS TIEING CHOKER ON STEEL THAT WAS BEING UNLOADED OFF A TRUCK. STEEL WAS BEING HOISTED OFF TRUCK BY A CRANE. THE LIFTING AND STRAINING TO TIE CHOKERS ON STEEL, CAUSED HIKM TO HAVE HEMORR HOID PROBLEMS.

EMPLOYEE WAS IN THE PROCESS OF DISCONNECTING THE HOOK OF A 1/2" DIAMETER WIRE ROPE WINCH CABLE ON A CATERPILLAR D6D DOZE BEING USED AND PULL BRUSH IN A LAND CLEARING OPERATION. DUE TO A TWIST ED CONDITION OF THE WIRE ROPE, AS THE EMPLOYEE DISCONNECTED THE CABLE HOOK IT SLIPPED FROM HIS HANDS, STRIKING HIM INT EH MOUTH AND DISLOCATING LEFT INCISOR.
