Showing all 39Employee stated that he and the section mechanic were lifting the miner cable up to hang it across an intersection when he felt a sharp pain in his right shoulder. Employee continued to work until 12-18-2013 when he had surgery to the shoulder.
Employee was helping move a booster pump when he felt a sharp pain in his back. He continued to work until August 1st when he became a lost time injury.
Employee was working on construction of an overcast when part of the wall fell causing the constructed portion of the overcast to fall injuring employees left knee. Employee was diagnosed with a torn meniscus on 3-18-12 and became a lost time accident at that time.

Employee stated that he was installing a mandoor in a stopping when the mandoor fell over cutting his right index finger. Employee received 8 stitches at the Middlesboro Hospital. Employee returned to work on the next shift.
Employee stated that he had a rib channel lying against the roof bolter when it fell over and struck him in the mouth requiring 2 stitches to his lip. Employee returned to work on the next shift. A safety meeting was conducted with employees to prevent similar occurrence.
Employee was loading roof straps onto the roof bolter when they slipped and cut his hand though his glove resulting in 6 stitches. He did not miss any work and returned on the next shift.
EE stated that he was changing a fitting on a shuttle car and went to remove a crib block, when he caught the index finger on his left hand, crushing the tip. Approximately 1 inch of the finger tip was removed surgically. EE returned to work on the next scheduled shift.
Employee was helping install track when something pulled in his shoulder. He went to the ER where the x-rays were negative. He continued to work until 11/15/07 when this accident became a loss time.
EE loading header boards by hand onto a flat car to be transported underground when he set the boards down onto his left index finger. He was transported to the hospital and recieved 4 stitches to the tip of his finger. EE ret'd to duty next shift and continued normal duties.
EMPLOYEE STATED THAT HE WAS WORKING BUILDING A SEAL WHEN HE TURNED WITH A SOLID CONCRETE BLOCK IN HIS HANDS AND STRUCK ANOTHER BLOCK WITH HIS RIGHT HAND. HE WAS X-RAYED AND IT WAS DETERMINED THAT THE TIPS OF HIS RIGHT MIDDLE FINGER AND HIS RING FINGER WERE BROKEN.
Employee stated that he was loading rock dust onto the scoop when he turned with a bag of dust in his arms and hurt his left knee. He continued to work his regular job and consulted a Dr. who performed surgery on 3-28-07 to repair cartlidge in his knee.
Employee was pulling cable off the reel of the shuttle car when he felt something pull in his lower back. He continued to work until 06/04/07 when he missed his first shift of work and this became a lost time accident.
As employee was lifting timber, he felt a pull in his groin. Employee had hernia surgury on 8/01/2006 which was his first day missed.
While stepping off a flatcar, employee claimed to have pain running up back. He became restricted duty on 3/29/2006.
As employee was working on scoop he dropped a pinion on his left hand injuring his 3rd and 4th fingers.
As employee was reaching for something on the roof bolter, he cut his left arm on a strap. Recieved 9 staples in his arm.
As employee was loading roof bolting machine with supplies, he felt a sharp pain in his lower back. The employee began missing work on 6/29/2005 due to this injury.
