He placed himself under the gathering head pan to push take-up in place with his feet. The chain broke causing him to be caught between the gathering pan and mine floor. He had fracture of nasal bone & 3 ribs, sprain of sacroiliac & pelvis, pneumothorax trauma & liver laceration.
EE was hanging a ventilation curtain and cut his left middle finger, requiring stitches, on a CAB brattice hanger.

EE was drilling mine roof, a piece of rock hit drill steel and caused steel to come out of drill chuck. The drill steel struck his left forearm. He continued working his regular scheduled shift thru 5/3/11.
Employee "ALLEGES" that while hanging a miner cable some foreign material (rock dust) fell into his right eye. Subsequent investigation revealed the employee actually had rust in his eye from an off the job accident. The employee's workers compensation claim was denied based on this fact.
