EE was replacing a lip shroud on an Esco dragline bucket. While removing the wedge that locks the lip shroud to the bucket, the lip shroud fell (10 inches) onto the EE's feet. The bucket was located in the welding shop. EE was wearing steel toed boots.

Showing all 24Employee and Co-worker were down Slope 3 attempting to set a suction hose into a sump. The hole in the ice was not very large and both miners had to hold the pipe above their shoulders to get the end into the sump. EE was behind Co-worker when they were handling the poly pipe and stated after they had the pipe set that EE slipped a bit and felt something in EE's back.
Employee tried lifting a 4" poly pipe that was plugged with mud to try and clear the pipe. When the employee dropped the pipe, they felt something was wrong with their lower back.
EE was cleaning a scraper tire fender. EE thinks that is when the left knee was injured. EE had a limp at the end of shift.
A mechanic was removing a park brake from a 993K loader. The park brake weighs approx. 300lbs. and shifted as EE was positioning for removal. The EE pinched their RT middle finger tip, resulting in sutures to close the wound.
Miner was cleaning grease with pre-solve on dragline walk. It was hot outside and they were sweating they took their glasses off to wipe sweat away from the Rt. eye. Miner forgot they had pre-solve on their glove, getting sweat and chemical in an eye. Antibiotic applied.
Miner was having problems with rt. elbow, appears to be from a previous surgery non-work related. On July 3rd the mine was informed by WSI the inflammation in the elbow was in a different location where the surgery took place year ago. This appears to have been cause, while pushing on a dragline cable during the re-socketing process in May. Currently on restricted duty
Moving fender from Kress 363 and had dirt fall off and must have got a piece in the eye on 09/27/2018. The miner tried to wash it out. The eye was still irritated on 09/28/2018 dirt piece need to be removed by Ophthalmologist.
Contractor janitor was retrieving an aerosol paint can from a cabinet and removing a wood lath from behind the cabinet when the cabinet and stand tipped over due to damaged leg striking the janitor in the back, causing a bruise to mid and lower back. Prescribed a days dose of Acetaminophen/Hydrocodone
Mechanic was removing a guard between the transfer case and engine on a kress coal truck. They removed 1/2 half of the guard, while handing it to the other mechanic they brushed against the other half embedding a piece of metal in the left elbow 1/2 inch with of human hair, one stich required for removal.
The Miner Mechanic was loading a traffic cone onto holder rod on the back of a Terex water truck and dropped the cone on finger creating a pinch point to the gloved left index finger. It created laceration. Two stitches and tetanus given.
The D10T Dozer operator was getting out the dozer man basket and went to remove a tiling spade from the basket, The spade was in a slot that holds the spade in place. EE reached over left side and grab the shovel. The shovel got stuck and EE pulled it again and felt something pop in lower back.
Miner was changing dozer tilt hose on 9/9/2015, when he felt some sort of dirt particle fall into his left eye. He tried flushing his eye afterwards. The eye was still irritated the next day and was brought to the clinic. The particle was removed and he was prescribed some Tobradex for preventative measures.
During concrete pour line reducer was plugged. When unplugging reducer clamp was removed. After removal reducer fell causing safety whip to tighten and slide up handle. Slid across fingertip and remove 1/2" of left hand ring finger.
The Miner left a note on his time card at the end of his shift 2/26/2010 hurt right rib laying on his rt side working on a motor no medical treatment 3/1/2010 The Miner said he was ok and incident report was filled then. 3/4/2010 he claimed re-injury himself after he coughed. 3/5/2010 he saw the Dr. he return to the next work day and put on lite duty for the week.
The Dragline Mechanic was disposing of a Metal race bearing approximately 15lbs dropping it 8" into a scrap metal bin. The Dragline oiler walked by the bin at the same time. A sliver of steal flew out striking the Dragline Oiler in the Rt. Elblow. The sliver puntured the skin and had to be treated with medication to prevent infection.
The utility crew was moving a section of 6" discharge hose. The hose was covered with a large amount of snow. They hooked a pickup up to pull it out from the deep snow cover. When they pulled the section out, a section of 6" plastic pipe was connected to it came out. This struck just below his Rt knee, restricting his activities for three days.
Employee was working on a cat engine with four 6 ton jackstands. The employee pulled himself up from the hoses on the top of the engine. One of the stands came loose striking the employee. Injury - contusion on right foot and left thumb and scrape on shoulder. New engine stands were built and corrections made.
Employee was removing a small section of hydrulic hose from a storage reel brand "19mm 3/4" LN USA SEA100r12 4000PSI MSHA IC-84/12 H" The section of hose recoiled off the reel striking the employee in scalp causing laceration needing 8 stiches
The ee was climbing up and down off the boom foot and sections to use an optic scope for leveling. the ee began to complain of pain in his groin area.
EE WAS REMOVING A DUST PLUG FROM A COM-LOCK TYPE GREASE COUPLING TO FILL THE BULK GREASER ON A CAT 992 G LOADER. THE SPIRAL WOUND WIRE RING THROUGH THE DUST PLUG HAD BEEN DAMAGED. WHEN THE EE PUT HIS FINGER THROUGH THE RING AND PULLED TO OPEN A PROTRUDING WIRE PUNCHURED HIS FINGER THROUGH HIS LEATHER WORK GLOVE.
EMPLOYEE WAS CHANGING A LIGHT BULB ON A FRONT LIGHT ON HIS DOZER AND WHILE PUTTING THE LENS BACK ON, THE LENS BROKE IN HIS HAND AND CUT HIS FINGER. THE LENS MAY HAVE HAD A CRACK IN IT CAUSING IT TO BREAK.
EMPLOYEE PILED UP COAL WITHT HE FRONT END LOADER AND THOUGHT HE SAW SOME COAL ROCK. HE GOT OFF THE LOADER TO CHECK IT OUT, REACHED OUT TO MOVE A CHUNK AND STRAINED HIS BACK.
BELIEVE THIS TO BE A RESULT OF AN OLD INJURY REPORTED ON 2-27-80 OF A TORN LIGAMENT IN HIS RIGHT KNEE. IT IS EXPECTED TIME LOSS OF 10 DAYS AND 2-3 WEEKS OF RESTRICTED WORK FROM THIS SURGERY.
