Showing all 22Employee was fueling and servicing a rock truck when EE was struck by a fuel hose and knocked to the ground.
EE had removed right side of the cap and started loosening bolts on the left side. Was removing the 3rd of 4 bolts when the cap assembly shifted. EE instinctively raised right hand to block the cap assembly from hitting them. The cap assembly hit EE's hand and punctured glove taking hand with the cap assembly causing the cut to right thumb/palm.
Hydraulic tank cover was removed with crane. The cover was suspended with lifting brackets. The technician mistakenly removed the retainer bolts for the sight glass guard allowing the cover to fall onto EE's left. The cover impacted the employees foot just behind the steel toe of EE's boot.
Employee was laying on their back replacing a fuel filter on a pick up truck. They loosed the line and gas spilled out onto their face. A small amount of gas got under their glasses and into the left eye. The eye was flushed & washed, they suffered no damage to the eye. Employee was prescribed medication that makes this event reportable.
Employee was attempting to install a wear plate on a loader bucket, the wear plate fell striking the employees right thumb , causing a laceration.
EE was ascending onto a dozer, once stepping upward the EE felt a pull in the calf (leg) muscle.
Getting boosters out of the magazine, a band used to keep a case of boosters closed stretched and hit left eye
EE was climbing down the ladder of the rock truck, when ee missed a step, causing the weight shift to right ankle on the following step.
Employee was removing a hydraulic hose from a threaded coupler joint, when the hose sprung back striking the employee in the right maxillary central incisor tooth, making the tooth loose.
Employee stated that while getting on the excavator "on employee's first step" while pulling self up, employee felt a pain in employee's left shoulder.
Employees were attempting to break down a power screen for transport. Employee was helping to hold a cylinder up out of it's pocket and as he flagged the loader man to raise the screen, the screen slipped. It fell off of the bucket and smashed his hand between the cylinder and it's pin point.
Technician was working under a 992G. Raised up and struck his head on the hydraulic tank, causing his hard hat to drive his safety glasses into his forehead causing a laceration. Required 6 stitches to close the laceration on the bridge of his nose.
EE an employee of Virco Inc. cut a tree that fell striking another ee. Requiring to be transported by ambulance to Bluefield Hospital with injuries to his shoulder and ribs. The accident took place on Saturday Sept 29, 2012 at approximately 2:45pm.
Employee was replacing coolant temp sensor on 992G Wheel Loader. When sensor was removed, hot antifreeze sprayed onto employee causing burns to his pelvic areas.
Repairing a motor grader and a tire shifted due to wet/muddy conditions and hit employee in the chest.
Employee was steam cleaning a D-10 Dozer. While cleaning the area around the battery compartment, the battery lid was dropped.

Employee was removing wear liners from dozer blade when the middle liner that was resting on the blade and ground slipped and slid forward pinning the technician's right ankle/leg to the ground.
