Employee was sliding rub rail out of the way. Rub rail slid off the block that was supporting it and EE's finger was caught between the rub rail and the block causing a laceration requiring 6 sutures.
Employee was loading an argon tank onto a scoop when EE smashed the tip of EE's right index finger against the light housing on the scoop.

Employee was trying to lift a small knack toolbox upon a 4-wheeler to move it up when as EE lifted, EE felt sharp pain in lower back. EE was taken by ambulance to hospital and taken off work due to the injury.
Employee stated: was lifting 1 bundle of roof bolter plates from the side of the bolter to the tray and felt pain in right shoulder. Went to ER, told to follow up with Ortho who placed employee off work for 2 weeks.
Bending 5 foot roof bolts to make it easier to install in lower mining conditions employee felt pain in left shoulder, mining height was 55 inches
Employee was bolting in the #5 entry, when removing the steel from the roof the finisher separated from the pusher allowing the pusher to come out of the chuck striking EE in the safety glasses on left side of the face, EE was taken off work at this time
Employee had finished setting bits on #1 cm 6322 and was picking up unused buckets of bits off c/m and felt sharp pain in left shoulder. EE didn't cease work or miss any time until 6-6-23 when EE had surgery to repair the left shoulder and was taken off work then.
EE was installing a draw head on a flat car, as EE was lifting the draw head over a ledge on the car EE felt pain in lower back. EE continued to work until 7-20-22 when EE was taken off work by the Dr and prescribed physical therapy.
Employee was removing a 5 foot bolt from the tray of the roof bolter when the employee struck left ring finger against the bolt head and the ATRS causing a simple fracture to the tip end of finger
Employee was helping change 1000' of belt when the blue rope got hung on bottom hanger. EE flagged scoop to stop and went over to free the rope when the chain holding sheave wheel broke allowing sheave wheel to hit EE in the left side. EE was taken to hospital and released back to work but now is off work. All scans and test was negative when done at hospital
Employee was pulling on miner cable felt pain in abdomen, did continue to work from 08/25/21 until 7-25-22 when EE had to have the place repaired due to increased pain in that area.
Down on EE's knees pulling fly pad off the curtain board when EE felt a pop in EE's chest, EE finished EE's shift then went to hospital next day, was diagnosed with strained chest muscles told to remain off work at least two weeks

Employee and three others were installing a transition roller on the new belt head installation. They put the roller on the rail and were sliding it into place when one side slipped off the rail allowing the roller to fall, hitting the injured on the top of the left foot, resulting in a fracture. Became reportable at diagnosis on 12/2/9. EE missed no work before then.
Employee felt pop in pec muscle when lifting themselves out of forklift. Was given off work 2 mths and prescribed PT, we ask this be charged as a OCC Illness due to no event, this would be no different than getting out of a chair, on 11-12-19 EE was taken to hospital the prior shift for chest pains while standing in track entry two similar occurrences in 2 days?
Employee was recovering 4" water line that was hung from the mine roof, as the employee was loosening a coupling, a chain that was supporting the water line came loose allowing the water line to fall. This caused the chain to unwrap itself from the water line striking EE in the face resulting in a small cut on the left eye lid and an abrasion on EE's cornea.
Removing a cam roller from 72" structure turned body to lay roller on ground, right knee hurt. Testing shows osteoarthritic changes, medial joint narrowing. EE was taken to ER on 4/30/19 released back to work, went to another ER & hasn't worked since 4/30/19. Prior knee Sx. Ask this be deemed an occ. Ill. not lost days due to no single event caused pain, history of knee problems.
Employee was handling high voltage cable and felt a pain in EE's back.
Loading track, member was helping load track rail onto a Fairchild 35C scoop. Load slid hitting employee in the right knee.
Employee was removing a set of run-through ventilation curtains from the board they had been nailed to and when employee jerked the second section employee felt a pain in employee's right shoulder.

While trying to slide the bottom of the fork into place the top came out of the groove and overturned, striking the employee on left leg, fracturing the tibia and fibula.

When one employee flipped a track rail over onto a track rail that was connected to the track tie the injured employee was in the process of lugging to the rail on employee's side, employee's hand was caught between the track tie and the mine floor.
As EE was putting some hyd. hoses into a scoop bucket a couple of 9' timbers that were lying in the bucket became dislodged and fell onto EE's right hand. X-rays at the ER were negative for a fracture. EE continued to have swelling so returned to the doctor on 5/17/2016 where X-rays revealed a small fracture and the doctor recommended physical therapy
Employee was moving shuttle car anchor and was letting down the slack that was hung by a rope hanger, the end of the rope hanger hook swung up hitting employee on the nose and when employee jerked head back the hook caught on the inside of nose resulting in 5 stitches, employee returned to work the next scheduled shift.

Employee was helping hang the miner cable and water line, he bent over and picked up the miner cable and felt discomfort in his groin area, he continued to work until 9-24-14 when he was taken off work and has been diagnosed with a hernia and is awaiting surgery to repair the hernia.
