Showing 50 of 59Employee was installing pin on a bridge when he caught finger under the pin. Employee did not miss any work however he did go to a medical facility to get an x-ray done and it was determined that it was fractured therefore making this accident a reportable.
Employee was attempting to pull a piece of rock off of the continuous miner when his hand slipped and he caught his left wrist on a sharp edge of the rock sustaining a laceration. He did not miss any work however he did receive eight (8) sutures thus making this report a medical.
Employee was directing the fork lift operator on changing batteries on scoop. He directed the fork lift operator to set batteries down; at the same time he reached for cap on battery jack on scoop. He got left thumb pinched between cap of jack and battery.
Employee had lowered the head of the roof bolter to remove drill steels. The finisher steel had stuck in the roof. As the employee was putting the pusher steel away, the finisher steel fell out of the roof, striking the employee on the ring finger of his left hand.
Employee was attempting to clean material that had built up between the head roller and the chute on the no. 11 belt with a roof bolt when the roof bolt got up against the belt thus causing an avulsion of the left index finger.
Employee had just placed rub rail onto frame of continuous miner & needed to move the cover plate in order to permit the pin to pass through the rub rail to secure to the miner, when the rail fell off the frame of the miner onto the top of employee's feet.
Employee twisted lower back while setting fibercribs. Employee did not miss any work however after seeking treatment for ongoing pain the physician placed him on restricted duty effective 12/12/2007 now making this report classified as a medical.
Employee was removing rock from the #1 bridge when he experienced pain in his lower back. Employee continued working until December 3, 2007 at which time he started missing work due to this injury, therefore, now making this a lost time accident.

EMPLOYEE WAS CHANGING OUT MINER BITS WHEN A SPRING CLIP THAT ATTACHES THE BIT TO THE BLOCK SPRANG BACK, STRIKING THE EMPLOYEE IN LEFT EYE. EE WENT TO THE HOSPITAL & WAS TREATED FOR A CORNEA ABRASION THUS MAKING THIS A MEDICAL.
EE, WHILE SPREADING HIGH CALCIUM LIME, APPARENTLY GOT SOME ON THE KNEE AREA OF CLOTHING NOT REALIZING THAT. LATER ON DURING THE SHIFT HE GOT HIS KNEES WET WHICH CAUSED A CHEMICAL REACTION.
WHILE EMPLOYEE WAS DIGGING ROCK IN COAL CHAIN, ROCK SMASHED HAND LACERATING FINGERS. EMPLOYEE RECEIVED 3 SUTURES AND PUT ON RESTRICTED DUTY, MAKING THIS A MEDICAL.
WHILE EE WAS WALKING IN MUDDY CONDTIONS DOING MINE EXAMINATIONS, EE EXPERIENCED PAIN IN BACK.

WHILE EMPLOYEE WAS LOOKING FOR AN OIL LEAK ON A ROOF BOLTER HE EXPOSED HIS LEFT HAND IN A PINCH POINT THEN OPERATED FUNCTIONS PINCHING LEFT MIDDLE FINGER.
EMPLOYEE WAS CHANGING BATTERIES ON THE SCOOP IN THE CHARGING STATION WHEN THE PLUGS APPARENTLY STUCK. WHILE TUGGING ON PLUGS THE ONE PLUG SUDDENLY DISLODGED THUS STRIKING HIM ON UPPER LIP.
EMPLOYEE WAS PUTTING BOLTER SUPPIES ONTO THE BOLTER WHEN HE EXPERIENCED PAIN IN LOWER BACK.
A ROOF BOLT WAS HANGING OUT OF ROOF, WHAT WE TERM A "CHANDELIER" WHEN THE EE STRUCK HIS RIGHT EAR ON THE BEARING PLATE THAT WAS ON THAT ROOF BOLT CAUSING LACERATION. EE RECEIVED SUTURES, THERE FORE MAKING THIS REPORT A MEDICAL.
EE WAS HELPING CO-WORKERS SET "I" BEAMS AND CRIBS WHEN HE EXPERIENCED PAIN IN LEFT KNEE AREA. EE CONTINUED WORKING UNTIL MAY 27, 2003 AT WHICH TIME EE STARTED MISSING WORK TO HAVE SURGERY PERF ORMED ON THAT SAME KNEE.
EMPLOYEE WAS PUTTING HIGH VOLTAGE CABLE ON SKID WHEN HE EXPERIENCED PAIN IN LOWER BACK. HE CONTINUED WORKING UNTIL 12/26/02 AT WHICH TIME THIS BECAME A LOST TIME ACCIDENT.
WHILE EMPLOYEE WAS ROCK DUSTING THE BELT LINE & THROWING BAGS OF DUST AROUND HE EXPERIENCED PAIN IN LEFT GROIN. HE CONTINUED WORKING UNTIL JAN. 20, 2003 AT WHICH TIME HE STARTED MISSING WORK T O HAVE SURGERY - DIAGNOSIS HERNIA.
EMPLOYEE WAS REMOVING A PIECE OF ROCK FROM THE BELT WHEN HE SUSTAINED A LACERATED FINGER THAT REQUIRED SUTURES.

WHILE LIFTING ROOF CHANNELS TO INSTALL THEM, EMPLOYEE FELT PAIN IN GROIN AREA. HE CONTINUED TO WORK BUT LATER WHEN THE PAIN DID NOT GO AWAY HE WENT TO DOCTOR AND WAS SCHEDULED FOR HERNIA SURGE RY ON 1/2/02.

EE WAS PUTTING VESSEL BACK TOGETHER WHEN THE PADDLE SWUNG STRIKING HIS LEFT KNEE. EE DID NOT NOTIFY US UNTIL 2-9-01 AT WHICH TIME HE REQUESTED A REPORT BE COMPLETED. ON 2-12-01, EE SAW A PANEL PHYSICIAN AND COURSE OF TREATMENT MAKES THIS REPORT A MEDICAL.
EMPLOYEE RECEIVED DIRT IN THE EYE WHILE WORKING UNDER A PERSONNEL CARRIER. EMPLOYEE DID NOT MISSANY WORK HOWEVER. HE GOT EYE MEDICATION FROM THE PHYSICIAN FOR TREATMENT, THEREFORE MAKING THIS A MEDICAL REPORT.
