Employee was helping recover belt structure. EE bent over to pick up a piece of structure and felt pain in the back.
Employee was putting locomotive on charge. While employee was plugging in the charger battery bell to the batter, employee felt pain in their back. Note: Employee did not inform mine management of accident until 11-29-16.
Employee was installing belt line. While threading belt line on stacker belt, employee was lifting on belt line and hurt right shoulder and right hip.

Employee was watering scoop batteries and the battery lid fell down cutting his fingers he received 7 stitches on his left ring finger and 9 stitches on his left middle finger. Come back to work the next day.
Employee was loading supplies on the roof bolter and cut the back of his left wrist on a roof bolt plate. He received 7 stitches. Came back to work next day.
Employee was roof bolting when the drill steel fell out of the drill hole and hit him on the left hand cutting his palm of his hand. He received 8 stitches, he came back to work the next day.
Employee was taking the cover off of the tail shift on a feeder. When he left it drop down on his right middle finger. He received 6 stitches then came back to work. No lost time.
Employee was hanging a roll of belt line when his right pinkie finger was caught between a chain and chain hanger.
Employee was roof bolting when the drill steel stuck in the roof when he reached to get it the drill steel fell and struck his middle finger on his right hand between the drill steel and the roof bolter cutting it he received 12 stitches.
EE was shoveling belt line when he pulled a muscle in his back.
EE was loading structures when a piece of the structure slipped out of his hand, cutting his left forearm. He received 4 stitches.
Employee was hanging roof bolter cable when the cable fell down striking him on the right shoulder.
EE was putting supplies on shelf. There was a utility knife blade in a box and it cut his right hand he received 5 stitches. Came back to work the next day no lost time.
Employee was picking up roof bolts from behind the roof bolter and a thin piece of metal fell off the bolter striking him in the left arm and cutting it. He received 5 stitches. He came back to work the next day. No lost time.
EE was picking up pot on the roof bolter and twisted his back.
Employee was roof bolting. When he was hanging curtain board he dropped curtain board. It hit his right thumb pulling muscles in it.
Employee was unloading structure off flatcar into scoop bucket when he mashed middle finger on right hand between structure and scoop bucket. 2 stitches, returned to work the next day. NO LOST TIME.

EE WAS RUNNING BUGY. HE STOPPED TO MOVE 4 5 GAL BUCKET OF OIL. WHEN HE PICKED ONE UP HE FELT PAIN IN HIS ABDOMEN.
EMPLOYEE WAS PUTTING A PLANETARY IN A MINER. HE RAISED THE MINER TO PUT CRIBS UNDER IT & IT FELL ON HIS HAND, CRUSHING LEFT 1ST & 2ND FINGER.
THE EE WAS HELPING MINER OPERATOR WITH CABLE WHEN A PIECE OF DRAW ROCK FELL BETWEEN BOLTS & HIT HIM.
THE EMPLOYEE WAS HANGING MINER CABLE WHEN HE FELT PAIN IN MID-LEFT BACK.
EMPLOYEE WAS PULLING ON CONVEYOR CHAIN ON THE MINER WHEN HE INJURED HIS GROIN.
