Showing all 47Opening the shop garage door and another employee ran over to help him lift it and employee said it took the weight of the door from him by surprise, causing him to hurt his lower back. Doctor took him off work for his back on 3/20/2015.
Bates Contractor was trying to move a piece of draw rock out of road and strained his left shoulder and right upper arm. He continued to work up until a doctor took him out of work on 2/10/15.
Employee was stacking block off of a pallet and hit his left knee on the corner of the block and felt pain in his knee.
3rd shift was belting back and the belt ends were spliced. The splice hit on top of top roller. Injured employee removed top structure and placed on right rib and left foot. Sharp edge on belt structure cut hole in boot resulting in a puncture wound to the top of the left foot, just above his big toe. Open fracture to bone on left foot just above big toe.
Employee was lifting crib block over his head and felt a sharp pain in his right shoulder. He continued to work up until 3/3/2015 when he had his pre-op and then surgery on 3/5/2015.
Employee was pulling slack in miner cable and felt pain in his back. He continued to work up until 1/2/14 when he was taken off work.
EE was changing batteries on the section scoop. When he went to throw the battery ball jumper onto the scoop, he felt a sharp pain in his lower back.
Employee was hanging miner cable and waterline for the scoop to go by and was moving a pump which resulted in a burning pain down his leg. He did not seek medical attention at that time. On 6/2/2013, he complained to experience pain and numbness in his leg and requested to see a doctor. He started missing work at that time.
While bending a bolt, bolt slipped causing a strain to employee's back
The employee was in the #6 entry when he attempted to push by hand, a wooden post on top of a Fletcher Mobile Roof Support. While pushing the wooden post the employee felt pain in his lower back.
On 3/20/2012, employee was throwing a loop of miner cable out of roadway. He felt a pull and burning in his groin area. The employee continued to work his regular job. On 3/30/2012, our company was notified that surgery was required to repair the hernia. Surgery was performed on 4/2/2012 and the hernia was repaired.
He was handling metal and a piece of slag cut his finger on his right hand.
While pulling feeder cable cat-head across the power center it slipped out of employee's hand and fell on the other hand mashing right hand between the cat-head and the power center wheel.
Employee and Co-workers were building cribs. The employees were passing the crib blocks to each other. The employee dropped a crib block and reached down to pick up the crib, when his co-worker handed him another crib. The employee's right hand was mashed between the crib blocks.
The employee bent over to pick up a metal H beam and moved the beam over to the rib. When the EE lifted the beam he twisted his lower back. EE cont. to work and did not rec. any medical treatment until 9/29/10. The EE ret. to his reg. duties on 9/30/10. The EE cont. to hurt and went back to the Dr. on 10/6/10. The Dr. placed EE on restricted duty on 10/6/10.
Employee was helping load parts to a retriever when he caught his left index finger between two of them mashing his finger.
The EE was making a splice on an electrical cable. After making the splice, he was attempting to hang the cable to get it out of the road way when he strained his back. Note: The EE did not seek any medical treatment for this injury at the time of the accident. He continued to work until 9/27/10, at which time he went to a Dr due to the continuing pain in his lower back.
EE was standing in front of a pile of rope bolts, when a bundle fell, striking him on the top of his left foot. EE went to doctor later in day. X-Rays showed a hairline fracture in left foot. EE did not miss any work and returned to his normal job the next shift.
EE was drilling for a roof bolt and tried to remove the steel from the hole when the steel got hung up. EE was pulling on drill steel when the steel broke free and the EE hit his right elbow on the drill pot.
Employee was shoveling belt. The employee told the witness that he was having chest pains. Employee stated that he threw a piece of rock against the rib and continued to shovel, then started having chest pains.
While closing 1st set of airlock doors, part of crew opened second set of doors causing 1st set to slam shut, catching EE's left foot.
EE was cleaning coal off scoop. While cleaning, the EE pulled coal block and fell down over smaller block, allowing the larger blocks to fall on leg and left foot.
Employee was rolling miner heads by hand to set bits when he pulled lower back and groin area. *Employee started physical therapy due to this injury on 4/29/08.
EE alleges he was loading roof bolts from scoop bucket to roof bolter when his right shoulder began to hurt. He was angry and began slinging roof bolts out of the scoop bucket. He grabbed several at one time & jerked on them causing the alleges injury. We are reporting this accident to meet the 10 days requirement but after our investigation is comeplete.

Employee was loading resin on top of roof bolter when he felt a sharp pain in his lower back.
Employee alleges he hurt his right shoulder while picking up cable out of roadway. This incident is under investigation.
Employee was helping change a shuttle car tire when the tire fell over striking employee on left knee.
While moving belt rails at the section tailpiece, EE felt pain in stomach area when he turned to throw rail against rib.

When installing rub rail on shuttle car employee bent over to lift one end and strained his lower back.
EMPLOYEE PUSHED OPEN A SET OF AIR LOCK DOORS WHEN THE WIND CAUGHT THE DOOR AND THREW EMPLOYEE TO THE SIDE.
EE WAS ASSISTING IN THE REMOVAL OF A LID ON THE CONTINUOUS MINER TO ACCESS A HYDRAULIC COMPONENT HE PLACED HIS HAND UNDER THE LID TO HELP RAISE IT WHEN THE LID FELL ON HIS HAND LACERATING THE RING FINGER ON HIS RIGHT HAND - 4 STITCHES.
EMPLOYEE WAS PULLING CABLE OFF SHUTTLE CAR REEL WHEN HE FELT PAIN IN HIS CHEST.
EE WAS PICKING UP TRAY (APPROX 5#) OUT OF TOOL BOX AND HAD LOW BACK PAIN.
EE WAS INSTALLING A SHACKLE ON THE MRS AND A PIECE OF PAINT CHIP FLEW UNDER HIS EYELID.
EE WAS TAKING POGO STICK DOWN, IT CAME LOOSE STRIKING HIM IN THE MOUTH AND BRERAKING A TOOTH.
EMPLOYEE WAS CHANGING A PUMP OUT AND WHILE HE WAS MOVING IT, HE FELT A BURNING IN HIS LEFT SHOULDER.
EMPLOYEE BENT OVER TO MOVE MINER CABLE AND FELT A PULL IN LOWER BACK.
