Employee stated that as he moved a shield, a rock fell from the canopy, striking him in the right leg.
On 4/12/05 completion of an investigation and review of medical records, indicated the employee suffered a second hernia while exiting a Wagner Scoop.
The employee was determined to have a hernia on the right side. He believes that it occurred on 02/07/05 while assisting the setting of Big John roof support. (4/18/05 per call - hernia repaired)
The employee attempted to cut a wire band on a bundle of wire roof mesh to separate the smaller bundles. The mesh sprung and then recoiled back into the employee, pinning him between bundles of mesh.
USING 2 COME ALONGS TO LIFT TILT CYLINDER FOR INSTALLATION. CYLINDER SLIPPED THROUGH CHAIN ON 1 COMEALONG AND FELL. LEFT THUMB WAS CAUGHT IN THE CHAIN OF THE OTHER COME ALONG CAUSING A LACERATION
EE STATED THAT AS HE TRIED TO TIGHTEN A CHAIN BOOMER IT SLIPPED OPEN AND STRUCK HIM IN THE MOUTH.
SEVERAL EE'S WERE ATTEMPTING TO MOVE A BELT SNUB ROLLER. THE ROLLER ROLLED ONTO THE EE'S THUMB.
THE EE WAS HELPING TO OPEN AIR LOCK DOORS. THE OUTER DOORS OPENED AND THE AIR PRESSURE SLAMMED THE INBY DOORS ON THE EMPLOYEE.
THE EMPLOYEE STATED THAT AS HE ATTEMPTED TO LIFT A ROCK OFF OF A CONTINUOUS MINER HE SLIPPED AND FELT A GROIN STRAIN.
WHILE HE ATTEMPTED TO MOVE MINER CABLE WITH HIS FOOT THE CABLE PULLED HIM INTO THE MINER.
AS HE ATTEMPTED TO LEAN ROOF MATS AGAINST THE RIB, THE MATTS TIPPED TOWARD HIM AND KNOCKED HIM DOWN.
ON 5/9/01 EE HAD SURGERY ON HIS RIGHT SHOULDER WHICH HE STATED WAS INJURED ON 3/28. HE STATED THAT AS HE ASSEMBLED A ROOF RUSS, HE FELT HIS SHOULDER POP.
THE EE STATES THAT HE WAS STURCK IN THE BACK BY STOPPING BLOCKS THAT HAD STUCK TO THE ROOF WHEN A STOPPING WAS KNOCKED OUT. THE EE WAS MOVING BELT HARDWARE NEAR THE STOPPING AFTER IT WAS REMOV ED.
THE EMPLOYEE ATTEMPTED TO USE A HIGH PRESSURE EMULSION HOSE TO WASH OF A SHIELD HAULER. THE HOSE MOVED IN HIS GRIP AND INJECTED EMULSION INTO HIS RIGHT LEG.
WHILE BUILDING A KENNEDY STOPPING, THE TOP RAIL FELL STRIKING THE EE IN THE RIGHT EAR. LACERATING THE EAR.
