On June 17, 2010 due to an occupational illness, as substantiated by medical records and reports, Jackie Thacker became unable to work because of the ailments described in the medical reports. This was not due to one single event at the mine, but repeated trauma over an extended period of time.

Showing all 35IEE and two other members were removing a 12" I-beam from the walkway and stopped to rest. When they picked it up again the EE felt a pain in his abdomen and neck. He was fitted with a C-colla r and was transported outside and then to the ER where he was diagnosed with a bi-lateral hernia.
While hanging a miner cable with a rope hanger, the hook of the rope hanger caught his thumb on his left hand. He received 8 stitches and was released to return to work.
Employee was shoveling on the slope belt and felt a pop in his shoulder. He continued to work while receiving physical therapy but did not show any signs of improvement and was scheduled for surgery on 12/01/2010. He continued to work until 11-30-2010.
While getting tools out of a tool box, tool box lid fell and caught the last 3 finger on left hand. Was transferred to surface and taken to emergency room and received 7 stitches.
Employee left work due to pain in low back, saw Chiropractor. RTW 3-4-09, work until 3-11-09, return to Chiropractor; taken off work until 3-13-09. 3-13-09 saw Chiropractor, taken off work again, RTW 3-17-09.
Employee at #5 entry, dropping RB cable, snagged sharp edge of pizza pan, cut through glove, contacted left index finger cause laceration. He was taken to the E.R. where they used stitches to close the wound.
Employee removing cover from mantrip. Cover slipped, slid across right hand cutting small finger on right hand. He was taken to the E.R. where stitches were used to close the wound.
Preparing to service equipment, was in bucket of scoop, stepped out, level ground-no debri, twisted left ankle. Splint ankle and taken to E.R. diagnosis; Subtalar Dislocation. Employee 6' 4" wt. 350 lb.
EE had Joy 10 SC at #3 entry intersection taking support rail from reel compartment. When end of rail fell onto mine floor, other end struck top of right foot causing injury. EE was taken to ER, x-ray was positive.
While loading rail onto rail car, section of 60 lb rail fell onto right foot, causing injury. Taken to ER, X-Ray positive.
Centers was helping to position the canopy on the roof bolter to align the bolts. During this procedure he strained his back. He was transported to Williamson Memorial Hospital by ambulance.
Taking RB plates from scoop to botler. Lifted 2 bundles from scoop, turned, tripped, drop plates onto side of left ankle. Taken to E.R. x-ray positve, fracture to left ankle.
EE WAS HELPING LAY 6"X20' AQUA WATERLINE AT THE BOTTOM OF THE SLOPE. HE DEVELOPED PAIN IN HIS LOW BACK. ON 5/2/06 MID SHIFT, EE LEFT WORK AND VISITED HIS FAMILY DR., WHO TOOK HIM OFF WORK DUE TO BACK STRAIN.
Employee was shoveling heavy wet material onto the #2 belt, and developed pain in the low abdominal area. He was diagnosed with an abdominal hernia.
ON MARCH 1 2005 DUE TO OCCUPATIONAL ILLNESS, AS SUBSTANTIATED BY MEDICAL RECORDS AND REPORTS, EE BECAME UNABLE TO WORK BECAUSE OF THE AILMENTS DESCRIBED IN THE MEDICAL REPORTS. THIS IS NOT DUE TO ONE SINGLE EVENT AT THE MINE, BUT REPEATED TRAUMA OVER AN EXTENDED PERIOD OF TIME
EMPLOYEE WAS LOADING 42" BELT STRUCTURE INTO SCOOP BUCKET IN THE BELT ENTRY. HE SLIPPED IN THE MUD WHILE LIFTING, CAUSING STRAIN TO THE RT. HIP & GROIN AREA.
MEMBER WAS PUTTING OIL IN THE MINER AND CUT HIS LEFT THUMB ON A SHARP EDGE ON A 5 GALLON OIL CAN HE WAS USING, RESULTING IN STITCHES.
EMPLOYEE WAS HANGING CABLE FOR BOBILE ROOF SUPPORT & RECEIVED A LACERATION REQUIRING STITCHES.
MEMBER WAS HELPING DROP MINER CABLE. WHEN CABLE DROPPED THE HOOK FROM CABLE HANGER CUT HAND REQUIRING STITCHES.
EE WAS SHOVELING ON THE #13 HEAD AND THE #12 TAILPIECE WHEN HE FELT A "POP" IN HIS BACK. HE COMPLETED HIS SHIFT AND VISITED HIS FAMILY PHYSICIANTHE NEXT DAY. HE WAS DIAGNOSED WITH A MUSCLE STR AIN AND EXCUSED FROM WORK UNTIL 12-26-02.
EE WAS HELPING INSTALL HANDING RAIL WHEN HE FELT SEVERE PAIN HIS BACK.
EE & 2 CO-WORKERS WERE WORKING ON THE 005-0 SECTION, HANGING MINER CABLE. EE'S ARMS WERE ABOVE HIS HEAD WHEN HE PLACED MINER CABLE AND WATER LINE INTO THE CABLE HANGER. EE IMMEDIATELY FELT PAI N IN HIS MID BACK. HE WAS TAKEN TO WILLIAMSON MEMORIAL HOSPITAL WHERE HE WAS DIAGNOSED WITH A HERNIATED DISC AT L4.
EMPLOYEE WAS CHANGING MINER BITS WHEN HE SLIPPED AND FELL A MINER BIT GRAZED HIS ABDOMEN RESULTING IN A LACERATION REQUIRING STITCHES.
EE WAS CLEANING THE B-3 BELTLINE WHEN HE STRUCK HIS HAND AGAINST THE BELT STRUCTURE RESULTING IN A LACERATION. EE WAS SENT TO A HAND CLINIC ON 06/06/02 WHERE 6 WERE APPLIED.
WHILE INSTALLING A NEW CONVEYOR CHAIN ON THE CONTINUOUS MINER A PIECE OF THE CHAIN BROKE OFF ANDSTRUCK EE IN THE LEG. MEDICAL TREATMENT WAS REQUIRED TO REMOVE THE FOREIGN OBJECT.
EE WAS MOVING A PIPE VULCANIZER. HE WAS HOLDING THE TONGUE OF THE VULCANIZER WHEN HIS LEFT LITTLE FINGER WAS CAUGHT BETWEEN THE TONGUE AND A PIECE OF METAL. EE FINGER WAS LACERATED AND REQUIR ED STITCHES.
EMPLOYEE WAS CHANGING THE SEAL ON A HYDRAULIC PUMP WHEN THE PUMP SLIPPED OUT OF HIS HANDS. EMPLOYEE RIGHT INDEX FINGER WAS CAUGHT BETWEEN THE PUMP AND ITS METAL HOUSING. HE SUSTAINED A FRACTUR E TO HIS FINGER AND A LACERATION THAT REQUIRED STICHES.
EE WAS PUTTING A TIRE ON SCOOP. THE TIRE FELL OVER, HE TRIED TO CATCH IT. HE STRAINED HIMSELF TRYING TO LIFT IT BACK UP.
EE OPENED DOOR ON ROCK BOX TO SEE IF ANY ROCKS WERE LODGE IN BOX. HIS HAND SLIPPED, GLOVE & COAT SLEEVE GOT CAUGHT ON SHAFT. THIS RESULTED IN A DISLOCATED FINGER AND A LACERATION TO THE HEAD W H4EN IT PULLED HIM AGAINST THE ROCK BOX.
EE WAS ATTEMPTING TO HANG MINER CABLE AND WATER LINE IN A CABLE HANGER. THE WATER SOURCE WAS ENGAGED ON THE CONTINUOUS MINER CAUSING THE WATER LINE TO JERK. THE CABLE HANGER CAME OUT OF THE SP AD NAIL AND STRUCK EE IN THE LEFT EYE.
EE WAS OPERATING A SCOOP THAT WAS LOADED WITH TRACK TIES. THE TRACK TIES BECAME ENTANGLED WITH THE PHONE WIRE AND EE EXITED THE SCOOP AND JERKED ON THE PHONE WITE TO FREE IT FROM THE TRACK TIE S. THE PHONE WIRE CAME LOOSE AND STRUCK HIM IN THE RIGHT EYE CAUSING A LACERATION TO THE RIGHT CORNEA.
EMPLOYEE WAS REPLACING A BLOWER MOTOR ON A ROOF BOLTER WHEN HE GOT HIS FINGER CAUGHT BETWEEN THE MOTOR AND THE FRAME OF THE ROOF BOLTER CAUSING A LACERATION TO HIS RIGHT THUMB THAT REQUIRED STITCHES.
EE WAS TAKING DOWN CABLES AND THE HYDRAULIC HOSE TO 78 HAND DRIVE WHEN THE CABLE FELL OUT OF THE HANGER STRIKING EE ON THE NOSE CAUSING A FRACTURE.
EMPLOYEE WAS TAKING DOWN CABLES AND THE HYDRAULIC HOSE TO 7B HEAD DRIVE WHEN THE CABLE FELL OUT OF THE HANGER STRIKING EMPLOYEE ON THE NOSE CAUSING A FRACTURE.
