Two employees were trying to take a link out of a drive chain, when a piece of metal broke off the chain link, striking an employee in the right eye, causing a deep cut on the top of the eyelid. Employee was taken to the emergency room, where he underwent surgery on his eye, and then was released from the hospital the next day.

EMPLOYEE WAS IN A HURRY, SO WHILE MOVING A MINER CABLE HE RAN HIS HEAD INTO THE RIB. THE IMPACT KNOCKED HIM TO THE GROUND AND RESULTED IN A MILD CONCUSSION AND NECK STRAIN.
