EE complained of pain in low back after uncoupling and handling belt structure. The accident occurred in the belt entry, there was no mining, and mining equipment has been removed due to the mine being permanently closed. A report was made online on 9/24/14 (attached). Upon reviewing 3rd quarter results it was noted that the report failed transmission due to an error of the writer.

Loading 4'water line on motor. Tried to caught end of water line falling off motor. Pulled something in right bicep. Continued working until 11/4/2013 surgery needed.

Changing cat planetary on roof bolter.

Removing center section from scoop and top pin in center section fell out hitting the left pointer finger, smashing it. He was taken to Greenbrier Valley Medical Center and received 6 sutures. He was given full release.

Employee got thumb caught between a crib block and miner frame while placing a block under frame of miner.

Switching seats in shuttle car, employee grabbed handle and when he pulled himself up he felt a pain in his shoulder

Employee helping put a shaft in SC, shaft slipped and smashed right index finger. Employee was administered first aid by EMT on section, came to surface signed a waiver and drove himself to ER where he received 5 stitches in right index finger.

EE was installing the side plate on a shuttle car. When he felt a sharp pain in his back.

pulling slack on 2" waterline, a clamp on waterline caused a laceration to right thumb of employee, employee went to Braxton county memorial hospital and received five stitches

Hanging a roof bolt cable and felt a pull in his back. Taken by ambulance to Summersville Memorial Hospital.

Employee was unhooking trailer from truck. He was unhooking the latch and the hitch came up smashing his finger. He went to Summersville Hospital and received 7 stitches.

EE was hanging miner cable along with co-EE. This EE was lifting & hanging the cable when he complained of back pain. He went to a chiropractor for treatment. This report is to abate ciation 8154329, which the operator intents to contest.

Employee was opening a bucket of plaster and some of the plaster splashed into his left eye. Employee says he was wearing safety goggles, management does not agree. Employee was transported to Summersville Regional Hospital and given time off from work.

Employee was in the process of getting the bendictor cleaned at eh #9 head. He shut the #8 belt off by the switch. He the got into the rock box, at which time the belt started again with him on it. Employee grabbed a belt chain which threw him off the belt. He was transported by ambulance to Summersville Hospital, received stitches and treatment for fractured ribs.

Employee was shoveling belt when he felt a pain in his lower back. He visited his family doctor for pain and continued to work until 05/14/2011 when he returned to doctor because of continuing pain. Doctor has placed employee off work until he can see a specialist.

Employee was loading ties on forklift, turned head, striking guard on forklift.

Laying track, had steel jacked up and then the steel fell off jack landing on right ring finger, received 4 stitches and a fracture to tip of right ring finger.

EE was setting bits on the Miner in the #6 entry inby spad #1334. He turned around to put an old bit in a bucket and took a catch in his back. He was transported to Summersville Regional Hospital by ambulance where the ER doctor placed him off work for a week to rest his back.

Setting spads and cut his finger on a roof bolt plate.

Employee was building a stopping. While lifting block, he noticed his back was hurting.

EE stated that he was removing a rock that was behind the scoop battery and cut his finger between the rock and the battery.

EE stated that he was placing a metal strap on the bolter to advance to the next row of bolts when his thumb slid through the hole of the strap which had a barb that cut through his glove. This resulted in a laceration at the end of his thumb.

Employee stated that he was helping to change batteries on the #1 scoop. The lid came loose from the open position and fell striking right index finger.

EE stated that he was changing a shaft on belt roller and it fell off causing hand to be pinched against roller frame.

EE was laying track and caught finger between rails.

EE stated that he was walking by a parked mantrip and the mantrip battery lid fell and struck his fingers, had hand on mantrip for support.

EE stated that he was tossing a damaged stopping panel into dumpster and cut index finger.

While putting jack walker back in, motor slipped smashing finger. Required 5 stitches.

EE had bent over to move a miner cable out of the way and felt a pain in his lower back.

EE had stopped to help the motorman move a sc tire that had vibrated off the flatcar and fell onto the track. While moving the tire he slipped and fell and the sc tire fell on his right leg. EE worked from 7/10/09 until 8/19/09 when he had surgery.

EE was loading bolts on bolter when he went to pull a metal band out from under bolts and it cut his palm.

Employee stated that he was helping another man load metal stopping panels onto trip to take underground. He slid his hand down the panel to turn it and cut his finger through the leather gloves he was wearing.

EE stated that he had climbed onto the miner to hang a reflector, as he was getting off the miner, he felt pain in his lower back. Note: EE stated that the pain started when he went from a standing position to kneeling, to get off miner.

EE stated that he was loading a roll of cable in a mantrip to take underground and mashed his little finger between a roll of cable that was in the mantrip.

On 8/6/08 EE was adjusting brakes and checking fluid levels on a mantrip at the surface motor barn. Upon completion of the checks, he lost grip on the access panel while lowering it into the closed position and dropped it approx. 18inches where it then struck EE's groin area.

Employee was loading belt structure on scoop. When he set the piece of structure down on his right index finger, it was mashed between two pieces of structure.

On 7/15/08 EE along with other EE's were in the process of folsing belt for a belt move when the roof bolt eye broke causing the sheave wheel to swing down and hit EE in the right arm. Sheave wheel was being ised to assist in the positioning of the belt and was attached to the plate eye w/clevis.

EE stated that he was helping unload tools at the no.5 belt head when water was turned on at valve where it hooks to main waterline. When line was pressured the nozzle hit him causing a laceration to his ear.

EE was unloading a bonder off of the mantrip and smashed his finger between the boncer and horn on the mantrip. He went to the hospital when he received 4 stitches in his right ring finger.

EE was looking at the reel on a shuttle car, when the lid fell down, causing him to strike his head on the shuttle car

On 2/15/2006 EE was leveling tail piece, while putting wedges under tail piece it slipped and caught his right thumb.

TAKING OVERCAST TOP OFF SCOOP, 2 MEN ON EACH END OF OVERCAST TOP WHEN THEY SLID IT OFF THE SCOOP. EE LET HIS END SLIP AND CUT FINGER.

Injured was helping a co-worker remove a piece of stuck drill steel from the bolter drill pot when a piece of metal struck him in the eye.

INJURED WAS PLASTERING A STOPPING IN THE RIGHT RETURN WHEN THE PLASTER SPLASHED ONTO HIS FOREHEAD JUST OVER HIS RT. EYE, RUNNING DOWN BEHIND HIS SAFETY GLASSES INTO HIS EYE.

EMPLOYEE WAS PUTTING 4" WATERLINE TOGETHER USING A CRIB BLOCK. WHEN HE PULLED BACK A CRIB BLOCK TO BUMP THE WATERLINE TOGETHER IT SLIPPED OUT OF HIS HAND & THE CORNER OF THE CRIB LANDED ON TOP OF HIS LEFT FOOT AT THE EDGE OF HIS METATARSAL BOOT. HE FINISHED HIS SHIFT THEN WENT TO THE DOCTOR.

EE WAS LIFTING A BOTTOM ROLLER AND FELT PAIN IN HIS LOWER BACK. HE DID NOT START MISSING WORK UNTIL 12-22-2003. AN ACCIDENT REPORT WAS NOT FILED WITH COMPANY UNTIL 1-9-2004 ON THIS ACCIDENT.

ON MAY 2, 2003, EE WAS UNLOADING A TIMBER MEASURING 82" LONG, 8 1/2" ACROSS WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK. INJURED CONTINUED TO WORK AND WENT OVER TO THE NEXT BLOCK AND CUT 8 MOR E BREAKER TIMBERS. HE WAS PICKING UP THE BUTS THAT HAD BEEN SAWED OFF WHEN THE PAIN GOT WORSE.

ON APRIL 9, EE WAS GETTING OUT OF THE SERVICE SCOOP. THE AREA WHERE HE WAS GETTING OUT WAS MUDDY. HE STATED HIS BOOT WAS STUCK IN THE MUD. WHEN HE STARTED TO TURN HE TWISTED HIS LEFT KNEE. HE DID NOT GO TO DR UNTIL APRIL 15, HE STARTED MISSING WORK ON APRIL 16TH.

EMPLOYEE WAS HOLDING THE HYDRAULIC DRILL WITH HIS LEFT HAND & REACHING DOWN TO THE MINE FLOOR TO PICK UP A WRENCH WITH HIS RIGHT HAND & FELT A CATCH IN HIS MID TO LOWER BACK AREA.

EE WAS DRIVING SUPPLY MOTOR. HE WAS TRYING TO EMPTY THE CAR & COULDN'T GET THE HANDLE MOVED. HE STRAINED SOME MUSCLES & BROKE A RIB.
