EE complained of pain in low back after uncoupling and handling belt structure. The accident occurred in the belt entry, there was no mining, and mining equipment has been removed due to the mine being permanently closed. A report was made online on 9/24/14 (attached). Upon reviewing 3rd quarter results it was noted that the report failed transmission due to an error of the writer.

Removing center section from scoop and top pin in center section fell out hitting the left pointer finger, smashing it. He was taken to Greenbrier Valley Medical Center and received 6 sutures. He was given full release.

Employee helping put a shaft in SC, shaft slipped and smashed right index finger. Employee was administered first aid by EMT on section, came to surface signed a waiver and drove himself to ER where he received 5 stitches in right index finger.

Employee was opening a bucket of plaster and some of the plaster splashed into his left eye. Employee says he was wearing safety goggles, management does not agree. Employee was transported to Summersville Regional Hospital and given time off from work.

Employee was in the process of getting the bendictor cleaned at eh #9 head. He shut the #8 belt off by the switch. He the got into the rock box, at which time the belt started again with him on it. Employee grabbed a belt chain which threw him off the belt. He was transported by ambulance to Summersville Hospital, received stitches and treatment for fractured ribs.

Employee was shoveling belt when he felt a pain in his lower back. He visited his family doctor for pain and continued to work until 05/14/2011 when he returned to doctor because of continuing pain. Doctor has placed employee off work until he can see a specialist.

EE was setting bits on the Miner in the #6 entry inby spad #1334. He turned around to put an old bit in a bucket and took a catch in his back. He was transported to Summersville Regional Hospital by ambulance where the ER doctor placed him off work for a week to rest his back.

EE stated that he was placing a metal strap on the bolter to advance to the next row of bolts when his thumb slid through the hole of the strap which had a barb that cut through his glove. This resulted in a laceration at the end of his thumb.

EE had stopped to help the motorman move a sc tire that had vibrated off the flatcar and fell onto the track. While moving the tire he slipped and fell and the sc tire fell on his right leg. EE worked from 7/10/09 until 8/19/09 when he had surgery.

EE stated that he had climbed onto the miner to hang a reflector, as he was getting off the miner, he felt pain in his lower back. Note: EE stated that the pain started when he went from a standing position to kneeling, to get off miner.

On 8/6/08 EE was adjusting brakes and checking fluid levels on a mantrip at the surface motor barn. Upon completion of the checks, he lost grip on the access panel while lowering it into the closed position and dropped it approx. 18inches where it then struck EE's groin area.

Employee was loading belt structure on scoop. When he set the piece of structure down on his right index finger, it was mashed between two pieces of structure.

On 7/15/08 EE along with other EE's were in the process of folsing belt for a belt move when the roof bolt eye broke causing the sheave wheel to swing down and hit EE in the right arm. Sheave wheel was being ised to assist in the positioning of the belt and was attached to the plate eye w/clevis.

EE stated that he was helping unload tools at the no.5 belt head when water was turned on at valve where it hooks to main waterline. When line was pressured the nozzle hit him causing a laceration to his ear.

EMPLOYEE WAS PUTTING 4" WATERLINE TOGETHER USING A CRIB BLOCK. WHEN HE PULLED BACK A CRIB BLOCK TO BUMP THE WATERLINE TOGETHER IT SLIPPED OUT OF HIS HAND & THE CORNER OF THE CRIB LANDED ON TOP OF HIS LEFT FOOT AT THE EDGE OF HIS METATARSAL BOOT. HE FINISHED HIS SHIFT THEN WENT TO THE DOCTOR.

ON MAY 2, 2003, EE WAS UNLOADING A TIMBER MEASURING 82" LONG, 8 1/2" ACROSS WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK. INJURED CONTINUED TO WORK AND WENT OVER TO THE NEXT BLOCK AND CUT 8 MOR E BREAKER TIMBERS. HE WAS PICKING UP THE BUTS THAT HAD BEEN SAWED OFF WHEN THE PAIN GOT WORSE.
