Employee was on a ladder helping another employee take down a come-along. The come-along slipped and fell while they were taking it down. The employee caught the heavy come-along with left hand. The weight made their left shoulder pop. The employee had surgery on 10/15/2025

IE was moving ASV forks by hand with another employee, the forks slid together smashing IE left middle finger in-between the forks. IE received 15 stitches for laceration repair.

IE was hauling pan line with hauler. IE went to adjust the sections of pan line IE was hauling, the top section of pan line slid off the bottom section. The pan line smashed both ankles and right leg, causing a fracture to the right tib/fib, left and right ankles. IE requires surgery to correct the factures.

IE was loading belt structure, when going to throw the structure IE right shoulder took all the weight. IE continued to work with no restrictions and was taken for evaluation on 11/7/2024 as well as 2/27/25 with no restrictions. IE had follow up evaluation on 4/28/25 where EE is placed on restricted duty.

IE was building a crib and was above the ground approximately 3 feet when the crib fell over. When the crib blocks fell approximately 3 or more cribs stuck the employee in various places on EE's body. One caused a fracture to T1 vertebrae.

Lowering chute into place, chute wedged and came down onto thumb resulting in amputation of tip of thumb. Originally restricted work only for 1 week. Then complications developed and employee returned home to WV for treatment. Injury changed from zero lost time to indefinite lost time.

Drilling supervisor was attempting to move a landing plate during the rig down process. After it was loaded onto the forklift they attempted to shift the plate which tipped to one side and caught the tip of their left ring finger resulting in localized cuts.

Employee was attempting to hang up the wash down hose by flipping it up onto the sims. The hose fell turning on the valve handle knocking off the employee's airstream helmet. The hose continued to whip striking the employee in the right eye ball causing a laceration.

While trying to unplug a 6" drain pipe for the washbay employee suffered a hernia. They were using a 1" hose as a snake to flush out the 6" line. The accident happened on 06/10/2013, he went in for surgery on 07/09/2013.

Employee while taking down monorail chain slipped loose on the other end causing the monorail to swing down pinching his left thumb between the two pieces of monorail, requiring 5 stitches to his left thumb nail bed. Employee returned to work his next scheduled shift on light duty.

Took employee in to have his right wrist looked at after a tire for the road grader popped off the rim while airing it up. The tire was in a protective rack and chained off. At that time the Doctor asked for a follow up exam, took employee back in on 05/29/2013 where he discovered a fracture.

Hauler door swinging shut went to catch it and punched the door unintentionally in an attempt to catch it. The accident happened on 10/12/2012 took EE to the doctor on 10/23/2012 after he complained of his hand still hurting where they found a fracture of the right pinky finger.

No Accident time determined. Employee allegedly strained his right knee while pulling a 14/3 electrical cable. This alleged accident was not reported to his supervisor as required. During investigation of alleged accident, his co-worker was unaware of any incident by employee. Operator concluded no incident occurred. Employer made aware of alleged incident on 6/29/12.

The employee was helping load a piece of 5/8 inch sheet metal into the loader bucket and move it to the surface shop. The employee had the bulk of the plate in the bucket when an edge of the plate hit the bucket and bounced back and cut the employee's leg. The laceration he sustained required stitches. He returned to his regular job. MSHA reportable.

The EE was on his knees wedging a corner piece into the bottom of a crib to stabilize it, when a crib block fell off the top, about 8-9'up and struck him on the head. The EE did not request to see a Dr. until 02/08/11 at which time an x-ray determined he had a fractured nose. EE RTW .

On 09-28-2010 we received substantiating information from Dr. that an employee sustained a new injury. On 08-26-10 the employee was helping install a bottom roller on BC-101, as he pushed the belt and lifted the roller, it caused his shoulder to dislocate. Restricted duty 08-27-2010.

The employee was helping change out belt rollers. He picked up and handed a top roller to another employee when he reported that he felt a sharp pain in his lower back. This injury was determined on 1/18/2011 by an Independent Medical Examiner to be a new industrial injury. MSHA Lost Time Accident 10/25/2010.
