EE was installing a roof bolt, ee reached for the fast feed lever, Hand ended up on boom. EE pinched ring finger between short arm and boom. (fracture and laceration)
While bending a roof bolt ee felt a burning in right shoulder. Then ee went to put the glue into the hole and ee felt shoulder pop. ( strained shoulder)
Was carrying oil cans in each hand, while walking. EE felt something in left knee. ***(Started missing on 03/17/2016. Orginal diagnosis was a cyst(non work related). After further test, as of 04/08/2016 changed diagnosis to strain.)

He was putting strap on miner cable, while bent over, a piece of material fell and struck him on lower back, causing him to hyper extend his knee.

He was working on shuttle car. He had lifted the cover to work in panel. He wedged cover in hog wire to hold it up. The cover fell and struck him on index and middle finger (Fracture of Index finger and cut on Middle finger)
He was installing a 84" roof bolt, he went to bend to bolt ,slipped and bolt fell out of hole and struck him on forehead . (received stitches)
He was shoveling coal out of a sump hole , while twisting, felt pain in back.(* Did not start missing until 11/12/2014 * )

while getting in shuttle car, there was screen wire hanging down. The screen wire caught his right ear and cut it (received 5 stitches)
While dusting the beltline he bent down to remove a damaged bottom roller. When dragging the roller he felt a strain in his groin area, causing him to have surgery to repair a hernia. ***Began missing work on 8/26/13***

He was replacing a piece of metal on front of shuttle car, he picked it up and turned to put it in place, when he felt his left knee pop. **(continued to work without restriction , until 8-31-12 when he went off for surgery)**
While bending an 84" tension rebar roof bolt, he felt a sharp pain in left shoulder. He continued to work full time without restrictions with pain until he was diagnosed with rotator cuff tear. ***He began missing work on 7-25-12.***
The crew was pulling a start box out of the crosscut when a braided steel cable got caught and pinned his leg between the cable and the corner of the rib. This resulted in a fractured right leg and ankle. ***Began missing work on 2-15-2012***
While roof bolting, he was inserting sections of the drill steel into the hole that was drilled. He felt a sharp pain in his right shoulder. He continued to work full time without restrictions until 06-07-12, where it was determined that surgery would be needed to repair a small tear in his rotator cuff.
