Employee was trying to hook up the broken chain on the slope of the wash box, when the chain broke free causing employee to slide down the slope to the divider and the chain struck him in the lower back. Employee worked up to 10/4/2012 and missed the first shift scheduled back on 10/8/2012.

EE was changing the ceramic lining on the plant cyclone, when the lining slipped, causing EE to pinch his right finger against the cyclone wall. EE received seven stitches to close the wound.
Changing belts and caught finger in mechanism. Skinned rt. little finger back midway between first knuckle and top of finger.
