Employee was trying to hook up the broken chain on the slope of the wash box, when the chain broke free causing employee to slide down the slope to the divider and the chain struck him in the lower back. Employee worked up to 10/4/2012 and missed the first shift scheduled back on 10/8/2012.

While taking a compressed spring out of a raw coal feeder, the EE suffered a fracture to his middle finger on his left hand when the spring released tension.

EE was changing the ceramic lining on the plant cyclone, when the lining slipped, causing EE to pinch his right finger against the cyclone wall. EE received seven stitches to close the wound.
