Employee lifted up draw head on locomotive and strained his back. He continued to work up until 9/16/13.
Employee was in the process of letting battery lid down while holding the lid by the handle. The lid slipped out of his hand catching the tip of his ring finger on his left hand resulting in 5 stitches.
Employee was lapping belt. The belt got fouled in structure and the belt clamp slipped off the belt striking him on the left leg on his shin. This resulted in a fracture (chip) of his tibia (shin bone).
Employee was bringing a 5 gallon bucket from lower steps to set on steps above. When he was doing so he felt pain in his lower back area. He continued to work until he went to the doctor on 4/17/12 and was taken off work.
EE WAS CHANGING THE REAR-END ON A 10-10 MOTOR OUTSIDE WHEN HE MASHED HIS RING AND MIDDLE FINGERS BETWEEN THE STEARING JACK AND THE REAR FRAME.
EE WAS CLOSING AIRLOCK DOORS BY HIMSELF. HE TRIED TO KEEP THE DOORS FROM SLAMMING SHUT AND WAS CAUGHT BETWEEN THE DOORS.
EE was trying to straighten a 6 foot torque tension bolt to push up into the hole, when he felt a something pull in his lower back.
EE had hand positioned on loaded rails, while chain ratchet was being secured around the rail. The ratchet had not started to tighten up and a piece of rail rolled over EE's left hand.
EE was putting a fork back on a fork lift when it droped and pinched his fingers between the fork and frame
EE was building a crib and was picking up a crib block and twisted around and felt pain in his lower back.
Employee lifted and was carrying a bucket of miner bits and twisted his back.
Employee was removing a stuck top roller while changing out the belt structure. He caught his right middle finger between the top roller and rail.
Changing out top belt cradle, pulled up cradle to lift out. Cradle had pressure on it from the weight of the belt causing it to mash his finger between the cradle and belt rail.
Employee was standing behind the miner head waiting as two members moved the cover onto the shear jack. The new members did not see his hand and dropped the cover on his hand.
Three minutes after the shift started, EE was putting his lunch bucket and a set of torch hoses and gauges into the back of a pick-up truck to take to the worksite and felt a sudden pain in his back, about the middle.
Member was in the bathhouse getting dressed for work, taping pants around boots. His utility knife was hanging open from his mining belt. When his right hand came around with the tape the knife cut his wrist.
EE was helping to remove a guard from the miner. The guard fell between the cat and pan. When trying to retrieve the guard, EE cut his hand on sharp metal.
EMPLOYEE WAS PULLING CABLE WHICH HAD YELLOW ROPE HANGER WRAPPED AROUND THE CABLE WITH TENSION ON HANGERS. EE TOOK ONE HANGER LOOSE; THE OTHER HANGER WAS BENT SOMEWHAT, HOOK-END CAME LOOSE & STRUCK EE IN THE NOSE.
EE was pulling one piece of metal that laying against the rib of the mine between forklift and mine rib of the metal fell and he tried to stop them metal fell on rt hand and left leg fracturing finger bruising leg.
Member was welding belt take-up, moved across beam and twisted his ankle at 003 section belt drive.
INJURED WAS PLASTERING STOPPINGS AND GOT "BLOCK BOND" INTO HIS GLOVE AND ONTO HIS SKIN. THIS LATER RESULTED IN A RASH OR BURN TO HIS RIGHT WRIST.
WHILE REPLACING A RETURN ROLLER ON THE #3 REFUSE BELT, THE EMPLOYEE FELT PAIN IN HIS KNEE & IT LOCKED UP. THIS HAS HAPPENED ON NUMEROUS OCCASIONS WITHIN THE PAST YEAR. EACH TIME, HE WAS ABLE T O POP HIS KNEE BACK IN PLACE. THIS TIME HE COULD NOT, REQUIRING MEDICAL ATTENTION.
WHILE PLACEING 8"X 8"X 3" TIMBER, MASHED MIDDLE FINGER BETWEEN TIMBER AND GEN. SKID, LT. HAND.
INJURED WAS SHUTTING A GATE AND CAUGHT HIS FINGER BETWEEN THE GATE AND A ROLLER THAT THE GATE SLIDES ON.
INJURED WAS PLASTERING STOPPING USING BLACK BOND. HE RECEIVED BURNS TO HIS ARM AS A RESULT OF CONTACT WITH THE B-BOND. HE WENT TO THE DOCTOR ON 1-4-02 TO SEE A SPECIALIST ON 1-6-02. THIS BE CAME A LOST TIME ACCIDENT ON 1-7-02.
INJURED WAS HELPING LAY TRACK. A FORKLIFT WAS USED TO RAISE A RAIL IN ORDER TO PLACE TIES ON THE RAIL AND LUG THE TIE, INJURED HAD HIS FOOT UNDER THE RAIL AS HE WAS LUGGING TIES. THE RAIL SLIP PED OFF THE FORK AND STRUCK ON HIS RIGHT FOOT.
TWO EMPLOYEES WERE GETTING A PUMP THAT WAS ON A PALLET BESIDE AN ELECTRIC MOTOR. AS THEY WERE PULLING THE PUMP FROM THE PALLET, INJURED'S RIGHT THUMB WAS CAUGHT BETWEEN THE PUMP & MOTOR RESULT ING IN A CONTUSION TO HIS LEFT THUMB THAT REQUIRED 3 SUTURES.
EE BENT OVER TO PICK UP BELT STRUCTURE AND FELT A PAIN IN HIS LOWER BACK.
A VALVE WAS STOPPED UP MAKING IT HARD TO OPEN AND CLOSE. EE GOT HOLD OF HANDLE & ROCKED IT BACK & FORTH. WHEN IT MOVED, EE'S FINGER WAS CAUGHT BETWEEN VALVE HANDLE & BOLT STICKING UP ON ANOTHE R VALVE.
INJURED AND ANOTHER EE WERE BOTH PITCHING CRIB BLOCKS AND OTHER EE PITCHED AND HIT HIM IN THE HAND WHILE HE HAD A CRIB BLOCK IN HIS HAND, CUT FINGER REQUIRING 6 STITCHES.
EE WAS ASSISTING IN REPLACE 2 200 HP DRIVE MTRS ON THE NO.2 DRIVE WHEN A CHAIN WHICH HAD BEEN SECURED TO 2 ROOF BOLTS PLATES CAME LOOSE CAUSING THE MOTOR TO FALL SUDDENLY & WAS DEFLECTED BY A PIECE OF ANGLE IRON DOWN ON EE'S LEG
EE WAS MOVING MINER CABLE OUT OF THE WAY. HE WAS BENDING OVER AND RAN INTO THE RIB WITH HIS HEAD. HE ALLEGED THAT HIS NECK WAS INJURED.
EE WAS LOADING 6' POINT ANCHOR BOLTS INTO A SCOOP BENDING AND TWISTING AND SUFFERED A LOWER BACK MUSCLE STRAIN. CURRENTLY RECEIVEING PHYSICAL THERAPY. HE IS CURRENTLY UNABLE TO WORK.
