Two employees were changing center can on a troughing roller. The roller was prepared for removal. As IE pulled the roller to remove it, his plan was to stabilize his end, allow the other end to drop onto the belt, then pull it out. As the roller dropped, the end of his left ring finger was caught between the roller and angle iron. The injury required sutures.

Showing 50 of 134Injured employee and co-worker were installing a return roller at the load out. Injured employee was holding a nylon strap that was attached to one end of the roller. The roller slipped from the co-workers hands - jerking the strap the injured employee was holding. Injured employee continued to work - had surgery to repair a small rotator cuff tear 12/22/2014.
I.E. was working on the fine refuse vibratory screen, he was climbing into work area he felt a "pop" in his left knee,and continued working. On 9/12/2013, the injured sought medical attention and was initially diagnosed with a strain of the left knee pending MRI results. On 10/3/2013 the injured was taken off work. His first missed shift was 10/7/2013.
A prep plant mechanic experiencing pain in his abdomen in March of 2013. He later had a congenital weakness of the navel (umbilical hernia) repaired on 4/2/13. Doctor's notes and definition included. Illinois. W. C.. covers this condition.
While doing belt recovery work, employee had removed the bottom rollers from their bracket on one side of the beltline. This put the rollers in a bind, when he pulled hard, the roller came loose, smashing the middle finger of his right hand between the roller and the belt rail. The employee was taken to the hospital and the finger was pinned and stitched.
While working underground on 11/5/12 EE felt pain in his left shoulder. He continued to work until he had MRI which shows possible tendonosis. See Attached
Employee was carrying a 6 foot timber with another employee when he wrenched his lower back. The top was low in the area. Employee continued to work until 11/8/12 when the Doctor took him off work and referred him to a neurosurgeon.
Employee was in the intake working on a ramcar. He had to lift the screen over the tram motor. Employee stated while standing on the side of the ramcar he lifted the screen and felt a pull in his lower stomach. Around an hour later he felt a small knot in his lower stomach. Employee finished his shift and was later diagnosed with a hernia. Employee is scheduled for surgery.
Employee was lifting miner cable over the scoop to move a generator. When lifting the cable over the center of the scoop, employee felt pain in his left shoulder. Employee continued to work until doctor took him off on 10/18/2012.
Employee was lifting his tool bag out of his tool box when he felt something in his right shoulder pull or tear. Employee came out of the mine and was transported to the Doctors office. He was later diagnosed with a torn rotator cuff. Employee continued to work until 10/22/2012 when he went off for surgery.
Employee was loading rock dust bags onto the scoop. Employee stated that when he turned to put the rock dust bag onto the scoop he felt pain in his back and right leg. A diagnosis of a lumbar disc was not made until 9/11/2012.
Employee was putting a tire in the back of #15 truck, employee was picking a tire up and twisted a little, he had a little discomfort at the time. Employee said that he misjudged how high to pick up the tire. Employee has a lower back and groin strain. Employee did not start missing work until March 1, 2012.
Employee was closing air lock doors on the Main North at 246 x-cut. He had closed the west door, when he went to close the east door he tried to catch it so it would not slam shut. His right hand got caught between the two doors causing a dislocated wrist and torn ligaments.
Employee was hooking up his material trailer to the back of the scoop. When he swung the tongue around to hook his trailer up, he smashed his left hand ring finger. The employee received a laceration which required 5 sutures to the finger.
Employee claimed that he was eating dinner at the dinner hole underground with his safety glasses on when he felt something in his right eye. Employee was seen that after noon by Harrisburg Medical Center and followed up the next day with an Optometrists where a piece of debris was removed from his right eye.
A mechanic was in the process of re-installing a cover on the roof bolter when it slipped out of his hands and struck him on both shins. He was taken to Dr. office where he received 6 stitches in his left shin. He was placed on restrictive duty by Dr.
Employee states he was helping the miner operator remove a rock from the machine. He braced himself and started to push, when he felt a "pop" and pain in his left calf muscle. Diagnosis was a torn calf muscle and the employee was placed on restricted duty.
Employee was carrying a 4X8 sheet of plywood and attempted to set it down in a rack when he felt pain in his left knee. Employee continued to work, but still had pain. He was taken off work on 8/16/2011. Subsequent review of medical data was performed to properly classify the injury/illness.
Extra drill steel fell and hit our employee on the back of the head and left shoulder.
Employee and others were hanging miner cable early in the shift. Employee is not certain of the time of any injury, but felt pain in his groin later in the day. Employee was diagnosed with an inguinal hernia from repetitive motion of entering/exiting the cab of his hauler and hanging cable. Employee was released for work, but began missing 3/12/2011.
Employee stated that he went to kick battery breaker off to lock and tag out for next shift. It was hard to kick off so I pushed real hard, something popped and burned in my left shoulder. Employee later went off work to have surgery. While off work, he had surgery on both shoulders.

He went for medical treatment and was told he has a Umbilical Hernia. These are there from birth and can come out anytime. This must be a illness.
Removing a machinery guard at tail of clean coal stacker. Guard hung on a bolt and then broke loose suddenly, pinching little finger on right hand between the guard and the concrete foundation.
Employee has had previous back pain and on this night he was working on a battery breaker box lid, putting it back on. Lifting the lid and positioning it caused his back to hurt more. He received medical services and taken off work for a MRI. Received the MRI today the 23rd, which showed lots of degeneration of several disc and arthritis (Illness).
Employee stated he had prior shoulder problems and when he pulled on a ventilation curtain it started hurting worse. The MRI shows Tendinosis, Joint Arthrosis. This is an illness that's caused by repetitive motions. This doesn't just develop in a few days.
Employee stated he was installing an 8 ft. bolt into the roof. When he went to bend it back straight his hand slipped and the bolt twisted straining his shoulder. Employee worked the next 2 shifts and started taking off on 5/23/2010. We was unaware he was missing work until 6/02/2010 and why he was missing. We did not receive a Dr's note until 6/03/10.
Employee was standing watching the Cont. Miner cable as it was being pulled out of the excess pile. The Roofbolter had started to tram to the face, when the injured was struck by the bolter cable that was hung to the roof. The cable struck him on the shoulder, arm, and mid-back. He worked until 04/25/2010 when he was taken off for testing.
Employee stated he was pulling on the bolter cable to get enough slack to hook on to move to another area. While he was doing this he felt pain in his lower right abdomen. Was taken off with a possible hernia.
Employee was in beside a Ram Car, in a sump, washing the machine off. He finished what he was doing, shut the water of and threw the water hose and valve up on the bank. The handle to the shut off hit just right and caused it to open. When this happened it sprang back under pressure and struck the person in the mouth. He's off work, lost 6 teeth.
Employee stated while bending a 6' roof bolt to get it in the hole, he pulled something (muscle) and the pain went into his back also. He worked the rest of the shift and stated there was even discomfort pulling drill augers out of the holes. He was treated by a family dr. and taken off work.
EMPLOYEE STATED THAT HE WAS BENDING AN 8' ROOF BOLT WHEN AFTERWARDS HE HAD A PAIN IN HIS LOWER BACK.
Employee stated he was removing the tongue on a Rescue Chamber, to install it on the other end. When he lifted up on it he felt a sharp pain in his right elbow. he continued to work until the 24th when he was taken off work by the doctor with a torn tendon on the bicep.
Employee was working in the Battery barn and was letting the lid down on this battery. He unhooked the rod that held it up and the lid fell shut. When it slammed shut it threw a small rock up and it struck the victim on his front tooth. It broke 3/4 of the tooth off.
Employee stated that he was bending down to pick up the miner cable to move it over out of the road, when he had a sharp pain in his lower left side of his back. He has had prior back problems and the MRI showed some bulging and at least two Liver lesions on the Liver. Also degenerative changes were noted. This all didn't just happen with him bending over.
Employee stated he was helping hang the Miner cable and his left shoulder started hurting. This shoulder has been bothering him for a while, but this time it seemed like it was worse. He continued to work until he was taken off by the doctor on 02/22/2010. Imsending the operative report to show that this was a illness. They had to trim a bone and spur off.
Employee stated he was attempting to install a 6'roof bolt. He had inserted the bolt into the drilled hole and started pushing on it to straighten it. As he was pushing away he felt pain in his lower back. Employee was treated in the ER and returned to work on transitional duty.
Employee stated he had just finished drilling a hole and was removing his two pieces of drill steels. These steels were stuck together and he was trying to pull them apart. He had to use a lot of pulling force to get them apart and when they did, the bottom steel jerked backwards and struck employee in the mouth. Causing Laceration to his lips. Stitches required.
Employee was trying to dislodge top section of pinner steel by pulling on it with hand, he had left bottom steel in the drill pod, when his right hand was pinched between the two sections of steel, causing a laceration to right hand requiring 10 stitches in it.
Employee stated that after his normal work shift, at his home, his arms started to hurt. The only thing he could relay it to was removing/handling concrete blocks from pallet in the process of building brattice. There is no distinct means for arms hurting, after doctor's visit he was diagnosed with tennis elbow/ tendonitis.
Employee stated he was helping move and hang the Cont. Miner cable when his shoulder which had been hurting started hurting worse. On the night of 12/06/2009 he left the mine with his shoulder hurting again and was taken off work on 12/10/09. MRI that was done showed Tendinosis.
Employe was trying to pull scubber door shut by pulling on chain attached to door. He grasped the chain with both hands to pull door shut. He felt pain in his stomach and belly button as he pulled the door shut.
Pulling miner cable out of perimeters, there was a wind row of gob, cable got tight and came over wind row and struck employee on the left ankle.
Employee stated he was helping more the miner cable and had some discomfort in his shoulder. Has had pain before.
Employee stated he was starting to push a roof bolt up in a hole and his shoulder started burning. Employee has just had surgery on this shoulder about 6 to 8 months ago. He didn't return to work until late Nov. 08. We are waiting on the scan/MRI. This is a re-occurrance of the prior Shoulder injury, stated the Dr.
Employee was removing a tie down strap from a tool box and was lifting a metal tongue up and pulling on the strap. Felt pain in his lower back.
Employee stated he was picking the miner cable up and walking it out of the roadway. After he got done he said it felt like his shoulder poped. Employee was unsure of the injury so have been waiting on the MRI to see if it wasn't an Illness.
Employee was throwing a crib tie under the belt when he smashed his right index finger between the crib tie and a bottom roller. He had a fracture and laceration to the tip of his index finger and required stitches.
Employee has had discomfort for a while in the lower abdomen. The dicomfort has progressed and with his normal activities on this day and the progression of discomfort he wanted to turn in a report. Employee has worked until 7/7/2009 when he entered the hospital for inguinal hernia surgery. This type is related to birth defects, other surgeries or wear and tear.
Employee stated he was helping move the Cont. miner cable over out of the way when a piece of tie wire hung in an ear ring. Not realizing it was caught in the ear ring he continued to walk on moving the cable. It ripped his ear lobe into two pieces. Medical services glued it at first but then he had to go back and get stitches.
Employee stated he was helping unload 3 ft. crib ties off of a trailer when another person grabbed one and threw it off and it glanced off of another one and hit him in the left knee. Employee has continued to work until 4/22/2009 when he was taken off by the doctor.
