Employee noticed something wrong with eye on way home from work. When ee woke up it was irritated and ee notice something stuck to eye. Employee received medical treatment for this injury at an optometrist office on 10/05/18.

Showing all 1312/1/17 EE & another EE was removing a piece of metal from chute when EE started experiencing pain in lower back. EE completed shift and filed an incident report that lower back was hurting, EE worked the following shift. EE sought medical attention on 12/2/17 and given full release. 12/7/17 went to different hospital and taken off work. Pain management for back since 8/2/12.
While attempting to remove solid concrete blocks from the raw coal crusher chute employee reached in to remove a block and another block fell striking employee on the hand, cutting right ring finger.
Employee, along with three other workers, was taking rollers off of a belt line. One of the rollers slipped and fell back into the rollers channel frame. Employee's thumb was in the channel when the roller fell. Employee received 4 sutures due to the laceration on right thumb.
Employee was relocating a 1.5 ton chain. When he went to pick the chain up, he felt a pain in left side and groin. Employee continued to work. Went to doctor on 3/13/12 and diagnosed with hernia. Surgery will be scheduled in near future. Employee is continuing to work full duties until the surgery.
EE was closing door on end loader, when door closed his hand was in door frame and door closed on middle finger cutting it. Resulting in laceration requiring stitches.
While installing a 1/2 nut & bolt in a rail over insured's head the nut slipped out of his hand & hit him in the front of tooth. Injured stated he chipped his tooth.
Employee was doing some housekeeping and while throwing some bags into trash container some of the chemical powder blew into his eyes. The left eye was irritated the worst. Received antibiotic ointment.
Member was helping take a pump apart. Moving the impeller to the side of the pump, it fell over landing on his left foot.
EE was taking engine compartment door off of dozer to gain access to starter. EE squated down to lift door and his right knee started hurting. First day away from work 5/4/06.
EE WAS CHANGING OUT BAD FLOOD LIGHT BULB, BULB BROKE, CUTTING INDEX FINGER.
EE WAS LIFTING A 42" IMPACT IDLER UPON THE FORKS OF A BOBCAT FORKLIFT, APPROX 1 FOOT FROM THE WORKING PLATFORM AND EXPERIENCED PAIN IN HIS BACK.
EMPLOYEE WAS MOVING A PIECE OF 6' PIPE. HE EXPERIENCED PAIN THAT NIGHT THAT GOT WORSE. THE DOCTOR STATED THAT HE HAD TWO CRACKED RIBS ON HIS RIGHT SIDE ALONG WITH SWELLING.
