Employee was installing roof bolts and T-3 channel and the channel slid off the ATRS and struck him on the right shoulder. Note: Employee did not start missing work as a result of this injury until 12/8/2011.

Employee had his left hand resting on the roof bolting machine divider between the roof bolt tray and glue tray and as he removed the bolt wrench from the drill pot and was placing it onto the roof bolt tray he struck his left middle finger with the bolt wrench resulting in a laceration.

EE was unloading a roof bolting machine canopy jack from the maintenance truck with a chain and hoist at the surface shop. As the jack was being raised, and EE placed his hand on the jack to guide it, the jack slid through the chain and mashed his right index finger between the jack flange and the hoist hook.

EE was reaching to get a roof bolt out of the tray of the roof bolting machine, when the bolt became stuck on other bolts. EE pulled on the bolt to free it and felt a pain in his right shoulder.

Making a belt move, pulling the conveyor belt through the tail piece, the clavis that attaches the wire rope to the belt puller became fouled up on the crossbrace of the v-scraper. He placed his foot on the conveyor belt to dislodge the stuck clevis. The clevis released and the belt puller and conveyor belt pulled through and caught his right foot between the cross-brace and belt.

Employee was operating the No. 187 continuous miner in the No. 9 entry and as the continuous miner was backing up, a loop of water line folded under the tail of the continuous miner. Employee stated that he stopped the continuous miner and reached down and was pulling the water line out of the way, when he felt a tearing feeling in his right shoulder.

EE stated he was dragging a section of line curtain when he felt a burning sensation in his left groin area. The incident investigation revealed that the line curtain was wet & muddy causing the curtain to be heavier than normal. EE was diagnosed with a hernia but continued to work until 8/27/07 when he started missing work as a result of this injury.

Employee stated that he was pulling up a 16' metal roof strap, lengthwise, and he was trying to hold the strap with his right hand and trying to get the resin glue into the drill hole with his left hand when he pulled something in his neck or left shoulder.

The investigation indicated that the employee was pulling on the continuous miner cable when he felt pain and burning in his right side. The employee started missing work as a result of this injury on 5-16-07.

The ee stated he was moving phone line up from a spool of wire. He was holding the spool of phone wire with his hands and while rolling the sire off the spool a sharp end of the wire cut his index finger and the top of his left hand.

EE stated that he was leaning over his tool box looking for a splice kit for the continuous miner. EE then proceeded to move a piece of metal out of the way, when he felt a burning sensation in his lower back and left leg.

On the morning of 9/25/06 EE reported at the end of his shift that he was having pain in his lower back. When ask if he had done anything to hurt his back. He stated that he had not, that the problem had been coming on for some time. When ask if he knew of any specific event that he was aware of to injure his back, he stated, that he did not know of anything.

EE stated that he broke loose the main 4" water line at E2 head & removed on section. He stated that as he pulled the waterline out of the way, he felt a pop in his rt shoulder. He then helped remove add'l 4" waterline from a scoop & place it along E2 belt. He stated that as he was pulling a section of this waterline that he felt another pop in rt shoulder & pain rt shoulder blade.

EE stated he was helping set a breaker timber approx 10 1/2'L & 20" dia. The timber had been cut to length & 2 co-workers lowered 1 end to the ground from the scoop & EE & another co-worker were holding other end of timber. W/out telling EE, other co-worker lowered the timber to rest on his knee, shifting the weight to EE who experienced pain in lower back.

EE was attempting to install an oxygen tank guard tube measuring 11" o d pipe w/1/2" thick walls x 34" long on the #5 mainten. truck. EE had it supported w/a 10" long piece of 6" channel, channel iron slipped out from under the pipe before the ee could tack weld it in place causing the channel iron to strike him on the rgt thumb & index finger and hyper extended his lft thumb.

EE stated that he and other ee were loading 48" belt conveyor structure (rollers) into the back of a mantrip. His left hand was caught between the belt conveyor structure and the canopy of the mantrip, as Other ee soved the belt structure into the mantrip. EE suffered a fracture and laceration to his left middle finger. The laceration required 4 sutures to close.
