Showing all 28Contract employee was picking up motors at PBS Warehouse. The motor was frozen to the ground and when Ee attempted to lift it and hit the pallet to break it free, the hook came undone and struck employee above left eye causing laceration.
Employee was loading nitrogen cylinder into service truck. While lifting the cylinder, the employee felt a sharp pain in lower back.
The injured employee was removing the right front wheel assembly on a Chevrolet 1/2 ton truck. The wheel assembly dropped approximately 12 inches to the ground and struck his right thigh and gave him a laceration.
The injured employee was unloading a piece of steel from the back of a delivery truck. Another employee was assisting him to unload it. The steel moved in an awkward way and stuck the injured person in the left knee.
An employee was spraying water into the discharge area of the filter shoot with wash down hose. A bolt that holds the filter disk in place caught his shirt sleeve and injured his right forearm.
Employee claims to have had an accident on 8-23-10 but did not report incident to the mine until 9-3-10. At that time the employee was sent to the panel of Doctors. On 9-6-10 the employee took vacation and company could not talk to him until 9-13-10 to report the accident.
The employee was climbing down the ladder located on the side of the shovel. The lower steps have ridges on them for traction so your boots don't slip. While dismounting the machine, the wedding band on his left ring finger caught the ridge on the step and cut his left ring ringer.
Employee was pulling welding leads up from the floor below. The welding electrode hit the employee in the mouth - chipping his tooth. Employee did not leave work because of the injury. He followed up with the family dentist to get his tooth repaired.
EE states he was holding a pc of steel to cut it and steel fell pulling his arm down causing pain in his right shoulder. EE did not report this injury or seek medical treatment until 8/7/09 when pain worsened. There is no est time associated with this injury. EE on modified duty pending orthopedic consult.
EE states that as he bent over to pick up welding rods, a 4' piece of angle broke loose over head and fell on EE, causing a concussion and contusions to the neck, head and (R) shoulder. EE is off work until cognitive evaluation is performed for stage 3 concussion.
Employee dropped steel beam on left foot; was approximately 40-50 lbs, 6' long x 13' wide and 1/4" thick.

Employee states he was putting PVC fittings into the back of a pick-up truck when he felt a tearing in his right shoulder. The employee did not notify the office of this injury until 11-15-08 (November 13, 2008).

Pulled muscle in right shoulder while installing turbo in a rock truck when it slid and he tried to stop it. He recently on Aug 17, 2006 was diagnosed with a torn rotor cuff and went off on Aug 18th to prepare for surgery.
Employee was helping dismantle a D9 dozer when the left push arm fell from the dozer strking his right leg and foot.
Employee reported striking his elbow on a motor shaft. He did not receive medical attention until 7/19/05 - Though he never informed the company of his treatment until 8-28-05. That's when he notified the company because medical bills came to his home.

EMPLOYEE NOTIFIED OFFICE THAT HE HAD SEEN A M.D. ON 3/24/03 AND WAS INSTRUCTED TO STAY OFF WORK FOR ONE WEEK. EMPLOYEE STEPPED FROM DRILL TO GROUND LEVEL AND FELT PAIN AT THE BALL OF HIS FOOT.
EMPLOYEE REPORTED ON 12/9/02 AN ACCIDENT BUT DID NOT SEEK MEDICAL. HE INFORMED COMPANY ON 2/21/03. HE WILL BE HAVING SURGERY ON 2/28/03. EMPLOYEE SAID HE WAS REPAIRING THE ENGINE ON A ROCK TRU CK WHEN HE FELT SHARP PAIN AS HE RAISED HOOD OF ROCK TRUCK.
EE WAS GREASING 966 LOADER WHEN HE STEPPED DOWN & TWISTED BACK.
TEAM MEMBER WAS TIGHTENING A BOLT ON THE ALTERNATOR OF THE OK 120 SHOVEL. HE EXTENDED HIS RIGHT ARM IN A HORIZONTAL POSITION TO REACH IN AND TIGHTEN BOLTS WHEN HE FELT A SHARP PAIN IN RT. SHOU LDER.
EE WAS REMOVING BOLTS FROM THE U JOINT OF, BOLTS, THE DRIVE SHAFT FROM THE WATER TRUCK. EE FELT PAIN IN UPPER RIGHT SHOULDER AS HE LOOSENED BOLTS.
EMPLOYEE WAS NOT PAYING ATTENTION OF HAND POSITION WHILE CLOSING A TAILGATE OF SITE TRUCK AND CLOSED GATE ON RIGHT HAND.
CREW WERE REBUILDING 2 SETS OF BELT LINE STRUCTURES. ONE SET WAS BEING HOISTED BY A CRANE AND THE BACK DRIFTED TO THE LEFT STRIKING THE SECOND STRUCTURE ON SUPPORTS STRIKING THE TEAM MEMBER ON THE LEFT SHOULDER AS IT SHIFTED. BRUISE TO LEFT SHOULDER.
