While loading roof bolt plates onto drill in walkway, EE twisted to pick up plates and felt a pop in his back.
On 4/9/12, EE was gathering materials to work on belt drive. He stopped by the tool hole to get tools. When he lifted the tool box to place on ride, he felt a sharp pain in his lower back. EE continued to work but pain got worse.
12/9/11 EE was helping to hang bolting machine cable. The cable reel hung up and jerked cable down catching EE from behind. The EE cont'd. to work and did not miss any work until 3/22/12. He had O.P. surgery on his knee on 3/22/12. This was his first day away from work due to this accident.
The miner was laying out roof bolts to bolt the last row of the 003 4-right crosscut. There were straps located in his tray, and as he brought his right arm back, he struck his right elbow on the straps. This resulted in a laceration to the miners right elbow requiring 12 stitches.
Employee was loading supplies to take to the roof bolter. A metal band had already been cut on a bundle of roof mats (straps) and as he turned to pick up more mats the sharp edge of the cut band struck his right forearm resulting in a laceration which required 10 stitches to close.
Contractor employee was removing the metal bands from a bundle of roof straps. The contract employee was pulling on the metal band when it came loose hitting him on the left forearm. The metal band sliced his left forearm requiring 3 stitches to close the cut.
EE injured back on 5/20/10 while setting timbers. EE refused to fill out accident form at this time. He continued working his normal shifts, on 5/27/10 he went to the doctor on his own.
The employee was helping the section repairman pick up tools and put into bucket. The employee lifted the bucket of tools to put them on a ride when he felt a sharp pain in his lower back.
Employee went to get his tool bag out of his tool box. When he picked it up, he pulled something in his right shoulder.

WHILE WALKING IN MUD, EMPLOYEE TURNED AND TWISTED HIS RIGHT KNEE WHEN THROWING A SHUTTLE CAR CABLE OUT OF THE ROADWAY. As a result of this injury, he started missing work again on 11/11/04. He returned to work with light duty on 2/4/05. Started missing again 2/7; results of MRI & doctors reports pending.
