Employee was helping with general cleanup duties while his co-workers were eating lunch. The employee was using a high pressure water hose to wash down the pugmill feed belt. The high pressure water hose he was using slipped out of his hand and the hose nozzle struck him in the chin resulting in a laceration which required 6 stitches.

Showing all 6Ldr oper. was helping change out training roller. Employee tried to pick up roller by himself from the dollie without employee assistance. This resulted in an abdominal strain. Employee felt that he was alright, but he went to the doctor on 5/10, 1 of his 4 days off per his schedule. The Foreman didn't find out that the doctor wanted the employee off for 1 week, until 5/14.
Adjusting pin on trolley needed to be removed to provide a more secure fit to the structure beam. During the adjustment the trolley arm came loose from one end striking the employee in the left side of the cheek. The employee received nine stiches as a result of this accident. No lost time.
EE WAS CLEANING PAINT OFF THE FEEDER & FELL THROUGH A HOLE IN THE FLOOR THATS USED FOR A LADDER.AS EE FELL HE REACHED OUT TO CATCH HIMSELF & HEARD HIS SHOULDER POP.
THREE EMPLOYEES WERE CHANGING THE RETURN ROLLER.ONE LIFTING THE WEIGHT OF THE BELT, THE SECOND ALIGNING THE BRACKET, INJURED LIFTING ROLLER. THE ROLLER BECAME LODGED ON A GREASE FITTING AND HE REMOVED HIS HANDS TO DISCUSS A FURTHER PLAN OF ACTION WITH HIS COWORKERS. WHEN HE PUT HIS HANDSBACK UNDER THE ROLLER IT BECAME DISLODGED FROM THE GREASE FITTING PINNING HIS FINGER.
EE WAS CLEANING PLANT WITH A WATER HOSE (200/250 PSI), WHEN THE EE NOTCIED A DECREASE IN WATER PRESSURE. EE WRAPPED END OF HOSE AROUND HIS HAND AND WALKED TO RELEASE A KINK IN THE HOSE. EE, HA VING HOSE IN HAND, RELEASED THE KINK IN THE HOSE RELEASING LOST PRESSURE AND BINDING EE'S HAND, RESULTING IN INJURY TO EE'S FINGER.
