As the employee was operating the Wagner 25X, #9, the door on the operators compartment came open. The employee stopped the machine and reached out to the top of the door and slammed it shut. As the door shut it smashed EE's right hand between the door and the cab of the machine. Stitches were required.

Employee was standing on top of a load of cattle guards attempting to hook up a chain to unload the cattle guards from a trailer when EE's foot slipped on a cattle guard and EE strained left shoulder as EE grabbed ahold of the wire roof mesh to regain EE's balance.

Employee smashed the tip of left middle finger between a bottom roller and a spreader bar while loading the structure onto a belt trailer in the 6th North belt line. X-ray indicated a fracture to the bone in the tip of the left middle finger.

While lifting a 50 pound bag of rock dust to fill the pod duster in the 6th North Development section, the employee strained EE's lower back as EE was tossing the bags of rock dust onto the pod duster.

Employee was building a Kennedy Panel regulator in the 6th North return. As EE was attempting to place a Kennedy flag on the upper corner of the regulator, the panel slipped down and cut EE's left index finger. The laceration was glued and employee was given a prescription by the doctor.

Employee was installing a 1 1/2 inch fire drop onto the 6" waterline in the belt entry at the 1st North tailpiece. The 5 foot long by 1 1/2 inch pipe fell and stuck the employee in the lower right leg and right ankle resulting in a fracture to the right leg/ankle.

Employee was attempting to advance the ventilation tubing in the #4 entry of the 1st North Development Section. As EE and another employee lifted the fiberglass vent tube, EE felt a sharp pain in lower back.

Employee was transferring a bantam rock duster from the bucket of a scoop into the bucket of another scoop when the employee's grip slipped and left thumb was cut by a wire that was sticking out of the rock dust hose that was attached to the duster. Employee receives 4 stitches.

Employee was building a cinder block wall for an overcast, as they opened the mandoor to travel thru the door, the wall tipped over and the employee twisted their right ankle in the mandoor frame.

The employee was troubleshooting an electrical problem on the continuous miner in 1st North Mains. EE had opened up a hinged protective cover. Another employee inadvertently bumped the hinged cover and it fell smashing the injured employees right middle finger resulting in a laceration which required 11 stitches.

The employee was dropping a vent tube from the mine roof when the nylon strap that was supporting the tube came loose and the employee caught Left small finger in the D-Ring of the strap and cut left little finger. 8 stitched were required.

Employee was installing a new haulage tram motor on the shear, as the employee moved a hydraulic hose out of the way, the motor shifted and smashed the employee's right index finger between the motor and the frame of the machine. 7 stitches were required to close the wound.

Employee had finished drill a 16 foot deep hole and lowered the drill mast on the roof bolter. The sectional steel got stuck in the hole and as EE reached for the starter steel, the section steel fell from the hole and hit EE's right hand resulting in stitches.

While loading a vent tube into the bucket of the 3 1/52 scoop, the employee smashed left index finger between the tube and the bucket of the scoop, causing a laceration which required 11 stitches.

Employee was dragging a piece of Tensar mesh out of the face when left hand snagged an insulated cable hook. The hook punctured through the skin between index finger and thumb on left hand. Employee receive a tetanus shot and a prescription for an antibiotic and was released to return to work with no restrictions.

Employee was operating the company #9 LST 25X in the 1st South Mains at XC 30. As employee stopped to cool the machine down, the door on the machine slammed shut and smashed employee's right pinky finger between the door and the door jam. 9 sutures were required to close the wound.

The affected employee was preparing to wash a piece of machinery off. When ee grabbed the wash-down hose and turned it on it came-out of hands and hit ee in the left side of the mouth. Giving ee a laceration and broke top and bottom dentures. EE received 6 stitches and antibiotics medicine.

Employee was building a Cement Block Stopping. He felt a pain in his lower back as he lifted a block above his head. The employee finished his work shift and also worked the next day. On the third day, 10/1/2015, he was hurting enough to request to see a Doctor. An MRI exam was done and he was diagnosed to have a "Disc Protrusion at L5-S1 abutting a nerve".

At approximately 10:00pm 3 employees were putting vent tube together. The male end of one piece was not going in the employee who got injured walked under the tube to retrieve a drill steel to push up on the male end. The end fell out striking him in the head.

The employee was installing aluminum water pipe. A piece of pipe rolled down off the stack and he grab it and lifted it up contact with the pipe cut his arm. Small laceration on the left wrist resulting in 4 stitches.

The employee was trying to get a can of anti-freeze out from under a pallet of Hydraulic oil, when a can fell off the top of the pallet smashing his hand. Causing a contusion and to small cuts to the back of the left hand resulting in 2 stitches. He returned to work on his next scheduled shift.

An employee was changing the fan motor of a Dozer. The motor was located in the top of the cab where there was no visibility. As he was loosening a bolt his hand came in to contact with the fins of the condenser. His left hand was lacerated between the thumb and the finger. Four stitches were needed to close the wound. The employee was released to return to work the following day.
