EE was using cable spooler to reclaim matrix coaxial cable. He was leveling the cable as it was being spooled on the reel. When the cable reached the coupling it pulled the cable out of his hand and through the reel striking him on the right hand, fracturing his small finger.

Employee and others were disassembling K&K structure. The bottom roller had been removed from one side of the stand by another employee, who flipped the roller striking him on the right cheek causing an inch laceration that required 5 sutures to close.

Stated: Employee was installing cover plate over fire suppression on scoop. He was holding bolt with left hand, cover with right hand. The vertical cover fell and caught his right thumb between the opening and the cover. It required sutures to close.

Stated: Employee was opening mandoor between neutral and intake. The handle was stuck so he hit the handle with his right palm. After the 3rd strike, the latch gave way. He raked the outside of the little finger on his right hand across a burr where the round stock had been cut to make the handle. He lacerated his finger requiring 10 sutures to close. He remains off work.

Employee was operating a diesel Getman. He dismounted the Getman to unhook from a loaded supply trailer. His glove became saturated with oil. When he attempted to mount the Getman his hand slipped striking the metal frame of the Getman.

Stated: Employee was helping hang miner cable across XC. They were pushing up on the cable to tie it close to the bolt plates. He felt a sharp pain in his lower right abdomen. He continued to work. As they were hanging cable for the next cut, he again felt the sharp pain. He was taken to the ER and diagnosed with a hernia which will require surgery to repair.

Stated: Employee was coming through mandoor by MN belt toward slope bottom. One large equipment airlock door, toward the slope bottom, was open. As she stepped through, someone opened the mandoor on the other large equipment airlock door. She lost her grip on the door and it mashed her arm between the door and the frame. She is still off work and is a lost time incident.

Stated: Employee was helping take down the rod used to hang a roll of belt for a belt extension. As another employee took the nut off the end of the rod to remove the collar used to hang the bar, EE had his hand on the belt core. When the end of the rod fell, his finger was mashed between the belt core and the collar on the other end of the rod.

Employee stated: He was opening a set of airlock doors at the slope bottom. He opened the first half and had his hand on the second half when someone opened the mandoor to the golf cart parking. The pressure change caused the first half to slam closed catching his hand between the two halves. The outside temperature was 15 degrees.

Stated: The employee was installing a roof bolt. The roof bolt plate had a curtain hook on it. He was holding the plate to keep the hook on the correct side of the plate while keeping upward pressure on his boom. He mashed his finger between the plate and the roof.

Stated: EE was cleaning off #17 roof bolter. The top lids were slid off to the side. He was trying to push the lids back onto the top & into place. While pushing on one of the lids, he strained his right knee. He finished his shift & worked a complete shift on 9/21. He works 4 days per week.

EE stated he and 6 other EE's were reclaiming a belt structure. The EE bent down to remove the roller off of the chain that was securing the load and a co-worker dropped a belt side rail onto his left thumb. This caused a fracture to his left thumb, EE was wearing gloves at the time of the incident.

Helping coworkers assemble FCT. Had been lifting conveyor flights across to a coworker as part of the assembly when he experienced pain to his low back region. Did not recall one specific flight that caused his pain, thinks repetition may have caused the muscle strain. Contacted the mine the following morning he was going to family Dr. his 1st day missed was 2/17/10.

He was cutting a piece of ventilation curtain off of a roll. He was standing bent over holding the curtain and pushing down on the knife. The blade met resistance at the overlapping curtain seem and he jerked the blade and cut the outside of his right shin. He was pushing down on the knife and his right leg leg was in close proximity.

After helping coworkers hand high voltage cable, pain was experienced to low back region. Contacted the mine on 1/20/10 and reported the incident. This was the initial date the mine was aware of the event. The investigation determined he was under a doctor's care and being treated for a muscle strain.

EE reported after dropping the bolter cable to tape spices, he was attempting to rehang the cable. When lifting the cable up to rehang over the cable hangers experienced pain to his shoulder. The following day, should became sore and experienced pain in the joint area.

Employee Reported: He and 3 co-workers were assembling steel arches. When placing the arch across a conveyor belt, the victim's right middle finger was caught between the arch and the steel belt frame. The extent of injury was an amputation of the tip of the finger.

EE was assisting installation of top rollers on new belt structure. While positioning roller, men on the other side of the belt pulled on the structure causing the top roller to drop into position. The 5th finger of his left hand was caught between the roller assembly and the top of the side rail. He jerked and pulled nail off finger, 3 stitches, started missing 5/12/09.

Employee reported. Making repairs to a belt tail peice frame. Using a come a long lifting a impact roller to placed in th ethroat of the tailpiece. the roller became stuck. He was pushing on the roller to move it over. It came loose and moved sideways suddenly. His right thumb was caught between the roller and the frame of the tailroller resulting in a laceration fracture only

EE reported: When he started to bend over to pick up a roof bolt supply, he experienced severe pain to his low back. There was no event that caused this pain. He was bending at the hips when he experienced the pain. He was transported to a medical facility.The x-rays were negative & he was given a prescription for pain meds. The mine is charging this as a work related illness.

Employee reported: Attempting to service a belt drive, as he stepped up onto an I-beam that the drive base was set on, he experienced pain to the back of his right knee. He reported the event to his supervisor at the end of his shift. Later that day at home, his knee began to swell and he sought medical attention for a possible strain.

EE was lowering a ram car battery lid in order to use it. He lifted the lid with his left hand & used his right hand to push the prop bar down. His left hand slipped on the lid & the lid lowered onto his right hand catching it between the lid and battery case. There was a laceration and fracture to the 4th finger of his right hand between the first joint & his finger tip.

When attempting to hand a continuous miner cable, experienced pain to his right knee. He alleges he stepped in a hole when hanging the cable. Continues to experience pain and swelling to the knee. Had MRI done on 2/26/09, determined he had a torn meniscus. Started missing schedule work day on 2/27/09.

EE reported: He and a coworker were bolting the face on #1 unit. The length of bolt being used was 8' long. Each bolt had to be bent prior to inserting into the mine roof. As he was attempting to bend a bolt, his left shoulder popped, resulting in severe pain to the shoulder. Started missing scheduled work days 1/28/09, became a lost time.
