An employee was cleaning out a chute on the #1 river belt. While doing so, the employee accidentally turned on the valve of a water hose and the water hose was inadvertently angled toward the employee's face. When the second valve was turned on, the hose sprayed water into the employee's eyes, causing an eye injury.

Employee was mounting locomotive 413 and as EE was ascending the locomotive stairs, the employee felt a pop in the right calf area. EE was evaluated at ER and given time off until follow up with Ortho. Ortho prescribed PT for a calf strain and degenerative conditions of the knee.

Employee was attempting to reposition a trough roller which was located inside a skid steer bucket. When the employee pulled on the roller they felt a strain in the left bicep. First Aid given, x-rays (negative). MRI ordered on 6/13/19 which revealed a slight tear in the bicep tendon. Employee referred to Ortho for further evaluation. No lost time, No restrictions currently.

Employee identified a broken crusher door pin at the rail loadout. After employee and the electrician removed the broken door pin, the employee held the crusher door in place while the electrician left the crusher platform to get a new door pin for replacement. The door then fell on employee's foot.

The employee, with the help of co-workers, was attempting to remove a bad roller. After loosening the roller, it tilted and pinched the employee's finger between the roller and the belt structure. This caused a laceration to the tip of the employee's left index finger resulting in 7 stitches.

EE aggravated his chronic back issues while mounting hill engine. EE stated that incident did not contribute to his chronic back pain, which is managed by meds. EE went to personal Dr. where he was put off until 7/1/15. EE had personal Dr. complete short term disability papers on 5/27/15, but this was rejected by HR. Case was processed as WC on 5/27/15.

Employee was shoveling coal near the truck scales and twisted his upper body resulting in a back strain. The employee had returned from back surgery with no restrictions the month prior (1/14/15). Employee didn't inform management until 3/1/15. Employee was evaluated on 3/2/15 and missed first shift on 3/3/15. This is a lost time accident.

EE maintenance Worker for KCT, on 04/22/2013 at approximately 2:00 pm, was reconditioning roller frames on a 226 D9R Dozer. EE was driving the equilibrium bar pin out, and as it was 1/4 inch from being out, ee placed a bolt in it, and was holding onto the bolt when the equilibrium pin came out and the roller frame fell and pinched ee left thumb.

Employee was performing pre-operational inspection of Loader and saw that A/C vent housing above his head in the cab was loose on one side. Employee attempted to snap vent housing back in place and shoved hand through the vent approx. 5 inches and made contact with fan blades resulting in lacerated and broken ring finger on left hand.

Employee had just dismounted a loader being repaired in the shop area and after placing both feet firmly on the shop floor the employee turned to his right and felt a pop in his left knee. Employee did not seek medical attention until 8/15/2011 and was placed off work for 1 week by his physician.

Employee was in process of pulling crabline out of water and placed hand in pinch point situation where his right wrist was pressed between the monkey line and boat, ultimately resulting in a bruised wrist.

Employee was placing a piece of scrap metal into a loader bucket for disposal into the scrap metal bin and in the process, pulled a muscle in his right bicep area, resulting in two days away from work.

Conductor mounted the cut of loaded railcars to protect the move to the hill, and as he mounted the railcar, pulled a muscle in his right shoulder. Employee was instructed by doctor to be off work and attend physical therapy for a potential muscle tear. Foreman was not made aware of incident until 11/30/2008.

While mounting a railcar, employee felt a pop in his left shoulder. Employee was given full duty release from local doctor on 5/14, but went back to the doctor on 5/17. The doctor on 5/17 put the employee off work, for further treatment, until June.

Employee was coupling an air hose to adjoin two railcars. When finished, the employee rose from the squatting position and felt a pop in his lower back along with pain. Doctor's report showed that the employee had chronic back pain and spasms. The employee had just returned to work on 10/17/05 from being off 3 1/2 months due to a non-work related back injury.
