ON 12/20 EE CALLED THE MINE SITE STATING HE HURT HIMSELF SHOVELING AND HAD A HERNIATED DISC IN HIS NECK AND WOULD BE OFF WORK. EE STARTED PHYSICAL THERAPY ON 12/20.
WHILE PULLING BOLTER CABLE WHEN OPERATOR WAS BACKING MACHINE OUT OF THE FACE AREA, THE EMPLOYEE FELT PAIN IN HIS LOWER BACK.

WHILE LIFTING THE MINER CABLE ON TOP OF A SHUTTLE CAR HE INJURED HIS BACK.

WHILE EE WAS CHANGING A RIGHT SIDE GATHERING MOTOR IN A EIMCO 2810-0 MINER THE MOTOR SLIDE TO ONE SIDE CUTTING THE EE'S LEFT HAND REQUIRING 5 STITCHES.
EMPLOYEE WAS PULLING MINER CABLE WITH SCOOP WHILE BACKING UP AND TURNING INTO A CROSSCUT DODGING LINE AND WATCHING FOR FELLOW WORKERS TO FLAG. INADVERTANTLY GOT HIS LEFT HAND OUTSIDE KITCHEN GETTING HIS HAND PINCHED BETWEEN PIVOT POINT. RECEIVING 2 BROKEN FINGERS AND THREE DISLOCATED FINGERS.
WHILE THE EE WAS LIFTING A CONVEYOR MOTOR FOR A SHUTTLE CAR ON 12-21-01, HE FELT PAIN INHIS GROIN. SOMETIME IN FEBRUARY HE WENT TO A PHYSICIAN AND DISCOVERED HE HAD A HERNIA. 3-5 WAS HIS LAST DAY OF WORK. HE HAD A HERNIA OPERATION ON 3-6-02.
