Employee was attempting to remove a broken bolt from a bucket pin on a front end loader. The chisel slipped as he was striking it with a hammer and the hammer struck his left thumb. The employee recieved a nondisplaced small distal tuft fracture of the left thumb.

Employee was using a box blade knife to cut a piece of conveyor belting during a repair to the conveyor belt. The knife broke causing him to cut his right thumb. Required surgery to repair a severed tendon. During mandatory drug testing due to the accident, he tested positive for Meth and Marijuana and was subsequently terminated.
While chipping at a weld, a piece of slag bounced off inside of the safety glasses and went into his left eye.
