EE was cutting off the inner race to change a bearing on the tail pulley of the screen feed belt. The splatter from the torch was landing directly onto a Chain Jack chain link. The Chain Jack was being used to horizontally suspend the tail pulley assembly. The chain link gave away which immediately caused the tail pulley assembly to release horizontally and pin EE's hand.

While cutting out a patch from belting, EE cut self with the utility knife EE was using. EE did not have gloves on at the time of the accident. A co-worker bandaged the left index finger that was cut. Co-worker took EE to the MedExpress where they put 5 stitches on the finger and allowed EE to return to work at EE's request.

I am a owner operator semi driver, i was dropping off a load of front loader buckets and had got to the unloading area and was proceeding to unchain the load. I was on my trailer using a bar to undo the binders so i could take the chain off and the bar flew off the end of the binder and struck me in the face. The result was a facial laceration that needed 5 stitches.

Employee was injured by accidental discharge of personal firearm on mine property while engaging in non-work-related activities. First responders were called and determined that the employee sustained a non-life-threatening injury to the leg. Employee was transported to a local hospital where EE received treatment and is expected to make a full recovery.

Crew was pressing dies into place on jaw using a portapower. Portapower unit broke, which caused the die to swing out and roll employee's foot, which caused the steel in the steel-toe boot to pinch foot.

EE was attempting to clear the 7x20 discharge chute when he was struck by several rocks ranging from 4" to 8" in diameter. He was struck on left shoulder & elbow.
