Employee was using a T-post driver when EE missed the T-post on one of the down strokes and the T-post struck the side hand-hold and smashed the tip end of EE's right pointer finger resulting in a fractured bone tip and stitches.

Employee was using a 36" pry bar to move a motor away from a fluid coupling to replace the spider. Employee pried one side and was attempting to pry the opposite side. Employee pushed the pry bar and felt a "pop" in RIGHT shoulder. Employee kept working. Employee felt alright until they tried to raise RIGHT arm and felt pain in shoulder. Employee was placed on "restricted duty".

Employee was in the process of removing the engine from 12019 hauler. The employee was using a pocket knife to cut a zip tie from a wiring harness. The employee was cutting in the direction of his left hand and the knife contacted the top first knuckle of the employee's hand causing a laceration

Using hammer to loosen bucket tooth, struck tooth adaptor on bucket and a fragment of the tooth ear broke off and went inside EE left leg.

Employee was utilizing a pick to clean ballast from beneath the rail for anchor installation. The bottom side of the pick head struck the base of the rail causing a small piece of shrapnel from the rail to strike the employee in the right thigh.

A mechanic was removing the left side final drive of a D10T Dozer when the male end of the 3/4" drive slipped out of the magnetic plug. The employee's left thumb struck the dozer's blade arm causing a fracture to the thumb.

EE was driving t-post for fence. While driving t-post he lifted t-post driver too high causing him to miss the t-post on the way down and caught his finger between t-post driver and t-post. Has to have stitches in left index finger.

The employee was removing the winch assembly from a D9R Cat dozer. As the employee pick up on the winch with the overhead crane, the winch assembly did not move. The employee used a 5 foot bar to try and pry the winch off. As he was prying up he strained his lower back. On July 18 2012 the employee started treatment with a Chiropractor making this injury a recordable.

The employee was assigned to secure a T-post using a 2' manufactured post driver to drive a 36" T-post further into the soil. The employee struck the T-post once, upon the second attempt he lifted the driver away from the T-post misaligning. This misjudgement caused the employee to be off by 1/4", which caused the contact between the T-post and his left hand, cut finger.

Employee was removing the discharge line from a water pump by cutting the 6" flexible hose with a folding razor knife when he cut his right leg just above the knee. Two stitches were required to close the wound.

The mechanic was installing a serpentine drive belt on a 3/4 ton Chevrolet pickup. As he pulled on a 1/2" drive, 16" long ratchet to loosen the belt tension, he felt a sharp pain in his left rib cage. He reported the injury the following Monday. He was taken to a local clinic where he was diagnosed with a left rib contusion and was placed on restricted duty.

EE was lifting and pushing a bar through the holes on his line boring fixture. The lift was a vertical lift from the ground. When the EE was making the final push upward, he felt a burning/tear in the lower abdominal area.

Employee in process of removing brake filter housing received laceration to upper lip. Employee was using a ratchet wrench on the bottom nut of the filter housing, pulling towards employee to break housing free. Once housing became free, the open end of the wrench came in contact with the employees right upper lip.

Employee was tightening a bolt on a front end loader when the wrench slipped causing him to hit the back side of his hand. At the time of the incident no medical attention was needed. On 9/26/13 the employee said he had a knot on his right wrist from the incident. Dr. done surgery to remove the knot on 4/7/14. So this injury went recordable today.

Supervisor was observing employee changing out teeth on the dragline line bucket. When the employee struck the tooth with a sledge hammer a metal sliver flew off and struck the supervisor in his right hand.

Employee was separating shim packs for upper bearing on main rotator of swing case #4 of a Marion 8750 Dragline. After heating the laminated shims, he was using a putty knife to work the shims apart. The putty knife caught and pushed the shim pack, slicing though his gloved hand causing a laceration to his left thumb and a cut tendon. *A 7% whole person impairment was issued.

Employee was raising a jack stand extension and it slipped pinching his left ring finger between the jack stand and the extension. The employee received a small laceration to his left ring finger that required 6 stitches.

The employee was cutting a pump flex-line with a utility knife. As he was cutting towards himself, the knife slipped and cut the inside of his right leg near the knee. He was taken to a local emergency room where steri-strips were used to close the small laceration.

Employee was trying to roll the front motor generator set #2 with a 5 ft. prybar. The bar slipped and the employee fell backwards against the rear motor generator set #1 mounting base striking his left back area. Employee sustained an occult rib fracture.

Employee was assisting with removing old teeth on the shovel bucket. When a fellow employee tapped on one of the teeth with a sledge hammer, a sliver of metal came off the tooth and struck employee in the left forearm requiring stitches.

On 8-25-09 employee exerted extreme force on a long handle wrench to remove a filter. Employee felt slight pain in shoulder not reported until 8-26-09. On 9/09/2009 at 6:30 am employee told supervisor his shoulder still had a slight pain. Physician did MRI and no restrictions on 09/10/2009. Second Physician visit recommended physical therapy that started on 10/06/2009.

Employee was assisting with high voltage termination in a BE2570 right main transformer room. While penciling the lead insulation of a 35KV cable with a folding razor knife, the knife cut through the insulation and stabbed through his glove and into the thumb side of the right palm. This resulted in a cut approximately one inch long. Employee is left handed.

Employee was changing the teeth on the backhoe bucket and struck the keeper pin with an 8 lb. sledge hammer. A sliver of metal flew off and struck the employee just above the left knee, requiring removal of the imbedded metal and 4 stitches.

Installing fender brace using sling chain. Brace was on ground standing on its end, chain attached w/slack. Employee holding chain in L hand, pushed brace down to ground. Brace fell and chain wrapped around L index finger and pulled tight.

EE's were changing out crossties on the rail. EE set spike on a new tie and as he struck it with the spike maul to drive it in, the spike popped up out of the tie and struck the EE in the mouth chipping a tooth.

EE was using a rail pick to clean out ballast from between ties under the rail to remove the tie plate. While doing so the ee accidently hit the rail with pick causing it to chip and a piece of the pick entered the ee's left cheek just under the skin.

Employee was in process of removing a nut from a gate hinge with a pipe wrench. During process of removing nut, in the motion of pulling on the pipe wrench to release the nut, employee strained his right shoulder. On 11/9/07 a MRI was given to EE, it was determined he would require surgery. Lost time began on 11/8/07.
