Employee was putting a door into an existing wall. After removing the block where the door was to go, the employee began striking the bottom of the wall with a hammer to straighten the bottom row. While striking the wall a block from a remaining top layer worked loose and fell, striking the employee on the left hand.

Employee was swinging a sledge hammer to knock the pin out of a track and felt a sharp pain in shoulder. Employee continued to work employee's regular duties and was sent for an MRI. Employee was sent to Ortho 3/15/23 and was diagnosed with a torn rotator cuff and placed on light duty. Surgery is scheduled for 4/18/23.

The employee was putting cable slings on the dolly arm. The employee's left ring finger was caught in one of the straps when the sling tightened. The employee pulled hand back resulting in fingernail being torn off.

The employee was using a pry bar in a downward motion and the pry bar slipped out. This resulted in pain in the employee's right shoulder. Employee was seen by an orthopedic surgeon and given a cortisone shot.

Employee was scraping a transfer chute with a long-handled scraper and hyper-extended shoulder. Employee continued to work normal duties until receiving an MRI and determining that the employee had a torn rotator cuff. Employee worked until surgery date of 9/10/2021.

Employee was using a come along to lift belt when the come along handle broke causing the employee to fall forward striking their left cheek off of the broken handle. Employee received a laceration from this that needed 5 stitches.

EMPLOYEE WAS INSTALLING A TRAM MOTOR IN A SCOOP. COMEALONGS WERE BEING USED TO LIFT AND POSITION THE MOTOR. THE MOTOR SLIPPED OFF OF THE FRAME RAIL ON THE SCOOP AND FELL INTO THE CRADLE. THE EMPLOYEE'S WRIST WAS POSITIONED IN AN AREA WHERE THE MOTOR STRUCK IT CAUSING IT TO BE PINCHED BETWEEN THE FRAME AND MOTOR COUPLER. EMPLOYEE WAS TAKEN TO HOSPITAL AND RECIEVED 6 SUTURES .

Employee was helping install a head jack cylinder when prying it into place the bar slipped causing them to slip and smash a finger under the pry bar against the pan of the miner. Employee received 9 stiches to close the laceration.

The injured employee and co-working were removing a metal canopy from a roof bolting machine. As the employees began to lift the canopy with a chain and ratchet to re-position the canopy, the chain and canopy slipped, allowing the canopy to shift, which resulted in an injury to four fingers on the left hand. (No broken bones). Cut glued together.

EE was using a large hammer to hit on a belt roller to break it loose from the steel structure. EE was holding the roller with their left hand. As EE was swinging the hammer with the right hand and hitting the roller, they missed the belt roller and hit a finger with the hammer. This resulted in a laceration to the left ring finger, requiring six (6) sutures to close the wound.

Employee was fixing a cat pad on haulage unit. Employee was driving the pin out using a hammer and a punch when a piece of metal flew off hammer striking them in the ear causing a laceration. The employee had the metal removed and received two stitches.

A miner was injured while striking a chisel with a hammer. A piece of rust/metal went under the injured miners safety glasses frame and contacted the left eye. Employee was wearing prescription safety glasses when the incident occurred. Prescription eye drops were prescribed because of the scratch on the left eye.

A miner was injured while changing a scoop universal joint. EE struck the universal joint cap and a sliver of metal became dislodged and struck EE in the abdomen. EE was transported to Conemaugh Emergency room to be treated. The doctor lanced EE's abdomen to remove the metal sliver and because of the lace, EE received three sutures.

The injured employee was attempting to remove a cord lash or nylon band from a bundle of roof bolts. The employee was using a utility or razor knife to cut the band on the bundle of bolts. As the employee used left hand to cut the banding material, the knife slipped and cut right index finger.

The employee was using a rock bar to pry on a bundle of metal straps to get to a bolt on the back of the roof bolting machine, when the bar slipped causing the employees right arm to come in contact with the corner of the metal straps resulting in a laceration to the right wrist when required (5) stitches or sutures.

Employee was helping change a cutter chain on the highwall miner. Employee was using a pry bar to line up the holes in the chain for the pin to be installed when his pry bar slipped and he smashed his finger between the pry bar and the shear arm. The employee received several stitches as a result of the incident.

The injured employee and Co-worker was removing the slack from the conveyor chain of a continuous miner. The employee had removed links from the conveyor chain and was re-assembling the pins and keepers in the chain. A sledge hammer was being used to drive the lock rings onto the pins when the co-worker hit the injured employee's left little finger with the sledge hammer.
