IE was using sledge hammer while making a repair on a roofbolter. As IE went to swing hammer, felt a pop in left shoulder area. IE has not missed any work. Reporting due to add'l treatment in form of pain shot given 02/21/20. Amendment: I.E is scheduled for surgery 5/12/20 & will be off work till released. Hasn't missed any work between inj date & surgery date.

While scaling down roof material, employee struck their right elbow on a wrench, chipping a bone in the elbow . It did not require any medical treatment.
Employee was cutting a hole in a belt dribble chute when something got in the corner of left eye. EE was prescribed prescription eye drops.
Employee was holding a hammer while another employee attempted to strike hammer to drive a gear coupling off. The 2nd employee missed the hammer head and struck the handle causing the injured employee's hand to strike the motor shaft. The employee received stitches to left middle finger.
Employee was cutting bands on screen wire. Band came back and cut left hand. The reason for the delay in reporting was completion of the accident investigation. EE had micro surgery on February 4, 2016 and became reportable at that time. EE did not miss any work. EE is currently on restricted duty.
Employee was removing tap from hole. The wrench slipped and he cut his thumb on the bit. His thumb required 3 sutures.

Employee was cutting the belt to make a splice. The blade on the belt knife broke and the knife was pulled into his left leg causing a 1 1/2 to 2" laceration
Employee was driving a spad into the rib. As he swung the hammer, he felt something "pop" in his right arm.
Employee was putting up sites in #2 entry of unit 3. He was driving a plug into the roof with a hammer when he felt something pop in his right hand and wrist. He started missing work on 5/31/13. This report was delayed waiting upon medical reports.
