Operator was standing on third rung of 6' step ladder while attempting to open a steam valve with a pipe wrench. They were using one hand on wrench and holding onto an adjacent valve with other hand when valve broke loose and they fell backwards off ladder onto concrete floor. Bruised heel of left foot and lower back. X-rays did not show any fractures.
