Employee was on a stand by HI-2 swinging a maul to open a drain valve when EE felt a pop in the right shoulder. EE was seen at Gulf Coast OCC Med and referred out to Ortho. MRI was done on 05-03-2019 follow up was on 5/7/2019. Results were right rotator cuff tear and bicep instability.
Operator was opening draw off valve at TK -34 where their left thumb was caught between the beater and maul. X-Ray was negative on day of incident. On 1/31/2019 after swelling went down they visited a hand specialist and was placed in a splint for 3 weeks.
Employee was trying to open a circulating line on T-6, in the process of swinging the maul they missed the valve and struck their right middle finger between the handle of the maul and a safety rail.
Employee was cutting tie wraps with a pocket knife. When the tie wrap broke the knife slipped and cut (L) middle finger.
Employee was nailing down a cloth on the #5 pan filter when EE hit left index finger with a hammer resulting in a fracture and a laceration.
Operator was trying to put #8 heater on the bad condensate line using a wheel wrench on the wheel valve. The wrench slipped smashing ( R) middle finger between the wrench and piping.
Maintenance employee was swinging a 10lb maul when they felt a pain in the (R) shoulder. Didn't report until day after injury. Negative x-ray was completed on 12-15-2017 at Gulf Coast remained problematic. Ortho MRI on 01/30/2018 revealed bicep tear and employee was restricted on 2/06/18.
Employees were descaling spool using a hand maul. The injured employee was bending forward looking at the pipe spool as the 2nd employee was swing the maul the maul struck the pipe spool then bounced off the pipe spool striking the injured employee on the left side of the face.
Employee was striking a chipping hammer with a ball peen hammer. While doing so, a piece of metal broke off of the chipping hammer striking the employee's left thumb. The metal punctured the glove and embedded in the employee's thumb.
