Another employee was using a maul to strike a metal wedge positioned between two flanges during disassembly work under a tank in the V2 area of the alumina plant. While striking the wedge, a fragment broke off and traveled approximately 15 feet, striking and penetrating the injured employee in the shoulder/neck area while the employee was working nearby.

Using a pry bar inserted into the notched holes of the barrel of a capstan fitting to open a gate. Employee applied force with the pry bar it bent and rotated causing the employee to lose their balance and fall onto the lime loading grating. During the fall EE's right arm braced the fall. EE's arm struck the grating and slid into the opening causing a fracture to the right ulnar.
Employee was using screw driver it slipped off the screw head. Employee was no wearing gloves, hands were sweating and the top of left hand caught a corner of the sheet metal panel they were working on causing a laceration.
