EMPLOYEE HAD BEEN WELDING ON A BREAKER WRENCH OF A HIGHWALL DRILL. WHILE CHIPPING SLAG A PIECE OF SLAG GOT INTO HIS RIGHT EAR. HE FIRST SAW A DOCTOR ON 2/25/95 WHO DIAGNOSED NOTHING WRONG. ON 3/1/95 HE COMPLAINED OF DIZZINESS & SAW A DOCTOR. A HOLE WAS FOUND IN THE RIGHT EAR DRUM. HE STARTED MISSING WORK DAYS ON 3/1/95.

A BUCKET PIN HAD PARTIALLY BACKED OUT OF THE DEMAG SHOVEL. THE EE WAS USING A HYDRAULIC JACK TO PRESS THE PIN INTO PLACE. THE JACK CAME LOOSE AND STRUCK THE EE ABOVE HIS RIGHT EYE. 5 STITCHES WERE REQUIRED TO CLOSE THE LACERATION.
