During typical pipe connection, employee pinched two fingers while utilizing pipe handling tools. The third and fourth fingers of the left hand sustained fractures and lacerations.
Employee was tightening air fitting on a pump with a crescent wrench when the wrench slipped off and employee struck his right index finger on the base of the pump, causing a laceration. Five stitches were required to close laceration.

EMPLOYEES WERE TRAINED ON SAFE USE OF KNIFE BUT FAILED TO UNDERSTAND THAT EVEN WITH SAFE DESIGN THE KNIFE COULD STILL CAUSE INJURY.
Employee was attempting to clean coal dryer chute when built up coal fines and water discharged and knocked employee off the work platform. Employee received fractures to his right elbow and nose, along with laceration to right eyelid.
While employee was using manual hoist to lift a metal tube, the tube struck him on his lower right leg causing a small laceration. Six sutures were required to close the wound.

EE FELT PAIN IN HIS SHOULDER AS HE WAS USING A SLEDGE HAMMER TO REMOVE A FAILED CONVEYOR BELT ROOLER BEARING. EE CON'T TO WORK ON REGULAR DUTIESUNTIL UNDERGOING SHOULDER SURGERY ON 11/5/03. HE WAS RELEASED TO RETURN TO WORK ON RESTRICTED DUTY.
EMPLOYEE WAS ATTEMPTING TO CUT AN OPENING IN PLASTIC TUBE WHEN THE KNIFE SLIPPED, CUTTING HIS LEFT THUMB. FOUR SUTURES WERE REQUIRED TO CLOSE LACERATION.

EE WAS USING A WRENCH TO REMOVE A FITTING WHEN HE FELT PAIN IN HIS LT. SHOULDER. ON 11/12/02, EE HAD SURGERY TO REPAIR A TORN ROTATOR CUFF IN HIS LT. SHOULDER. HE BEGAN MISSING WORK AT THAT TIME.
EE WAS REMOVING LARGE ELECTRICAL CONDUIT FITTINGIN PREP PLANT WITH A PIPE WRENCH WHEN FITTING SUDDENLY LOOSENED AND HE STRUCK HIS RIGHT RING FINGER AGAINST DRYER MOTOR. SURGERY WAS PERFORMED O N 1/9/02 FOR TRIGGER FINGER SYNDROME. EE RETURNED TO WORK ON RESTRICTED DUTY.
EMPLOYEE WAS SHOVELING WHEN HIS SHOVEL HOOKED ON BELT FRAME CAUSING HIM TO STRAIN HIS BACK. EMPLOYEE WAS ASSIGNED TO RESTRICTED WORK ACTIVITY BY HIS PHYSICIAN.
EMPLOYEE STATED HE WAS SHOVELING WET ROCK DUST WHEN HE STRAINED MUSCLES IN HIS RIGHT CHEST.
EMPLOYEE WAS USING A PRY BAR TO HOLD UP A HOSE SO HE COULD REPAIR A LOOSE FITTING. THE PRY BAR SLIPPED PINCHING HIS LEFT INDEX FINGER CAUSING A LACERATION AND CONTUSION. HE REQUIRED 4 SUTURES. STICHES TO CLOSE.
EMPLOYEE WAS IN THE PROCESS OF REPLACING AN OIL FILTER. HE WAS USING A 3/4" RATCHET TO REMOVE THE FILTER HOUSING. HE EXPERIENCED SOME DIFFICULTY GETTING THE HOUSING DISASSEMBLED AND WHEN IT LOOSENED HE STRUCK HIS UPPER LEFT ARM AGAINST A MOUNTING PLATE.
EE CLAIMS THAT WHILE ATTEMPTING TO OPEN A PRIMARY REFUSE ELEVATOR DRAIN WITH A METAL ROD HE FELT A PAIN IN HIS BACK.
EMPLOYEE WAS USING A RACHET WRENCH TO TIGHTEN A BOLT WHEN THE WRENCH SLIPPED OFF THE NUT AND THE PALM OF HIS HAND STRUCK A VLAVE CAUSING A LACERATION. SUTURES WEE REQUIRED TO CLOSE THE LACERA TION.
